This is in regard to Order No. ************* to HO Medical for a*box of masks. The order also included a request for Walmart Masks but*that part of the order was cancelled by Walmart and a refund was provided.**Although you reported that the rest of the order was delivered on Friday evening,*May **, I never received it. I live in a senior facility where all packages*and mail are delivered to the front desk in the lobby of the main building*so there is no chance of misplacement. The package just never got there.**Now I have a confession to make. On your address form, there are two divisions.*On the right is the residential address. On the left is the "preferred delivery address."*Temporarily I had somehow omitted the actual street address on the left side So only my*name, apartment number, city, state and ZIP were on the "preferred delivery address." Before*I noticed that and corrected it, I sent a number of orders. In spite of this blatantly incomplete address,*all the orders involved have successfully arrived except for the masks from HO Medical. The*orders that arrived all had the correct delivery address* apparently they were smart enough to check with your*records where the correct address was easily available.**Of course I tracked the package. It was taken by FedEx which handed it off to the U.S. Postal Service which*"delivered" it to a "mailbox." It certainly was not my mailbox.**Although this mix up is mostly my fault, others contributed to it. First, your computer program*should have rejected my incomplete address. On computerized forms that I have often had to fill*out, the slightest mistake inevitably results in rejection. Here the program failed to pickup an egregious*error. HO Medical should have communicated with you to obtain a more complete address which,*of course, you have on file.**What to do about all this I will leave to your discretion. But I do want to thank you*for this opportunity to explain the situation in my own words.
