Use this page to automatically get your PUK code once signed in to your AT&T account. Use the login credentials you created for your account and click Sign In.
If it doesn't automatically give you the PUK, go to Troubleshoot & Resolve and follow the on-screen instructions. Once you have the PUK code, enter it on the screen. Note that you may be given the PUK1 and PUK2. Enter the PUK1 if it's requesting the first PUK code, or the second one if it requests the PUK2.
Note: If you have a prepaid device, you may be able to find the PUK code in the original SIM card packaging.
Another way to get your PUK code is by contacting AT&T directly. Call the number shown in the screenshot and be prepared to verify your account information. You'll also need your AT&T wireless number, as the representative may need it to locate your account and retrieve your PUK code.
If you're using a Motorola device and the screen does not display "Enter PUK", you may need to enter 05 before entering the PUK code. Once prompted, enter the 8-digit PUK code and tap OK.
After the PUK code has been accepted, you'll be asked to create a new SIM PIN. Choose a PIN that is secure but easy for you to remember.
Once you have your PUK code, make sure you enter it carefully. If you enter the wrong PUK code too many times, your device may display a message such as "PUK Blocked, Call Operator." This indicates that the SIM card has been permanently locked and can no longer be unlocked using a PUK code. In this case, the SIM card will need to be replaced.

Were you able to obtain the PUK code? AT&T may have unlock requirements for your device. In some cases, the device must have been purchased more than 60 days ago and be fully paid off.
