Hello**My Enrollment Number for Safelink Service: ***********I recertified using the online portal and forgot to add my utility bill. My Safelink Service was de-enrolled by mistake.*I have attached - once again copies of my recent Utility Bill (PGE) and copies of front and rear of my Medi-Cal BIC card.*I am disabled and rely on my service which is currently deactivated. Please can you reactivate my service as quickly as possible.*Raymond Trevor Martin***** Bryant Street Apt *** SF CA ******Date of Birth: ***********Please Contact Me If You Need My SSN to Reactivate Phone: URGENT. Thank you.*Safelink number was *** *** **** BUT I bought a TracFone Smartphone and the number was changed to *** *** **** - this number is the number used for my Safelink Service. I do NOT use the original number *** *** **** though this is the number on mailing corespondence..
