T-Mobile will require you and the new account holder to verify your identities. You should therefore have photos of each of your government IDs, in case they are requested.You should also have personal information and contact information for both you and the new primary account holder.Make sure that you know the answers to your security questions, if any.

Go to T-Mobile's home page.

Click on the My Account link in the upper right corner of the page and sign in to your account.

Make a copy of your account number, and also any other information that you think might be important to have.

Go the T-Mobile coverage map and enter the ZIP code for the new primary account holder.

Check that T-Mobile offer coverage in the new primary account holder's area.

Make sure that this will all work out before taking the process too far and finding all your effort was wasted.

There is no other way to change the primary account holder, because of security concerns. While it is true that you can manage account roles and user permissions with the T-Mobile app, it is not capable of switching primary account holders. That is a high security operation that requires human supervision.
