Visit the Lyft "Help" page by going to Lyft.help.com.
Find the horizontal search bar on the Lyft "Help" page. It says "Search keywords or questions" and has a magnifying glass to its right.
Tap on the search bar. Doing so will outline the box, reveal a blinking cursor, and make your phone's keyboard appear.
Type "deactivation" in the horizontal search bar, but don't press the return button on your phone's keyboard.
The second search result that appears should say "Deactivations - Lyft Help." Click on this link. This will reveal the information underneath the search results.
Click on the information underneath the search results list. Doing so will cause the search results list to disappear, fully displaying the "Deactivations" page.
Read through the "Deactivations" page. There are several sections to read, including "Why do deactivations occur?," "Deactivation process," and "Appeals process." While you should read through each section carefully, pay special attention to the "Appeals process" section since you're interested in getting back your account.
At the end of the "Appeals process" section, you'll find a sentence that says, "For more information on how to appeal a deactivation, visit this article." The word "article" is a link. Click on it to go to the "Appealing permanent deactivations" page.
Read through the entire "Appealing permanent deactivations" page carefully.
When you're done reading the page, look for the link you need to submit an appeal. Under the "How it works" section, there's a sentence/link directly below the header that says "Submit a form to appeal a permanent deactivation." Click on this link.
Read through the entire "Things that can help you" page.
Look for the "I still need help" button. Click on it when you find it, which will cause a form to appear.
Fill out the form completely. Make sure to attach any supporting documentation if you have it.
Review the form to make sure it's correct. When you're ready, click the "Submit" button to send in the form.
