Name on the account, email, password, phone number, date of birth, and address

Zoosk will permanently disable accounts that have been compromised or involve a payment that has been disputed by the credit card issuer. Subscriptions can be transferred to new accounts that contain a new email and credit card/payment info, but in the case of chargebacks, a new account is not an option.
