To schedule a driver evaluation in Virginia, individuals typically need to be referred by a court, law enforcement, or a government agency. You will need to complete the "Application for Certificate of Title and Registration" (Form VSA 17A). Bring the death certificate of your husband, the current title, and proof of your identity.
