The form can help your friend obtain a replacement certificate of naturalization or certificate of citizenship. Please note that there are supporting documents, fee, and potentially biometrics, needed to be submitted along with the application. However, even for the duration he is without the citizenship document, his status as a U.S. citizen is not revoked. Please note that possessing a naturalization certificate is not the only proof of U.S. citizenship. A U.S. passport is also proof of U.S. citizenship and can be obtained by applying through the U.S. Department of State.
