I have faxed letters in attn to the Dispute Dept. on Monday Nov.*, ****. Informing that I called on Oct **, **** to report card lost or stolen. Told the card was canceled and would be mailing me a check for the balance remaining on card. Called Oct **, **** to make sure it had been processed and sent. Then was told that it never had been canceled or blocked. Finding that my card was still being used. Then filed report with Dispute dept. I faxed photo copy of receipt of my last time of using card on Oct **, ****. *Also included in fax a photo copy of my state I'd along with change of address letter and all correct contact information. *I would like to request some kind of documentation that everything is being processed on sending me new card and at what address being I sent address change information along with some kind of confirmation of filing Dispute.
