At Humana, all patients receive a member ID card that includes the name of their primary care physician. You should only visit the primary care physician listed on your card — if you see another doctor, they may not provide a routine checkup unless you update your primary care physician. Log in to Your Account

Enter your login credentials and click Sign In. You can click Forgot Username or Password if you don't remember your login credentials.

Select Change Your Primary Care Physician

Once you are already on the member dashboard, you can select Change Your Primary Care Physician if you wish to see another doctor for a checkup or treatment.

Find a New Doctor

After clicking Change Your Primary Care Physician, a new window that lets you find a doctor will pop up. You can look for a new doctor by specialty, zip code, or distance. If you find your preferred doctor, just click Select.

If the new window that lets you find a doctor doesn't pop up, just go to your Active Coverage and use the Find a Doctor link on the Active Coverage page or the Find a Provider tool below the page.

Confirm the Selection

The last step is to confirm that you want to change your primary care provider or physician. After confirmation, you can review the change in your dashboard. Humana will update your records and send you a new member ID card with the updated primary care physician.

Use the Humana Primary Care Physician Change Request Form

Patients can also use the Primary Care Physician Request Form to change their doctor or provider, and mail or fax the completed form to Humana.

The fax number is 1-800-633-8188. If you wish to mail it, use the P.O. Box address on the form.

If you don't have the complete name of your primary care physician, check your member ID card. Don't forget to state the reason for the change before mailing or faxing the form. Once received, Humana staff will verify and check the availability of the physician before updating your records and sending you a new member ID card.

Call Humana Directly

You can also call Humana directly should you have further questions about changing your primary care physician. The website also has a chat feature you can use anytime. If you call, do it only during business hours, which are from 7 am to 7 pm EST.
