If you have an insurance plan with Humana and have decided that you could do better elsewhere, then you need to learn how to cancel your plan. If you want to cancel your plan before its effective date, the procedure is very fast and easy. If you want to cancel it after the effective date, which is technically called "disenrollment," the procedure is a little more involved.

What You'll Need

You can disenroll from your Humana plan during the Medicare Annual Election Period, from October 15 through December 7.If you have a Medicare Advantage or Medicare Advantage prescription drug program then you can also disenroll during the Medicare Advantage Open Enrollment Period from January 1 through March 31.If you are eligible, you can also disenroll during a Special Election Period.Circumstances that may qualify you for a Special Election Period include but are not limited to: moving out of your plan's service area, losing your employer's Medicare coverage, qualifying for Extra Help, and qualifying for a Special Needs Plan because of a qualifying chronic health problem.Only a member, a member's legal guardian, or someone with a member's healthcare power of attorney can disenroll that member from a Humana plan.

Check your enrollment verification letter for your plan's effective date.

If your effective date has not yet come and you want to cancel your plan over the phone, go on to the next step.
If your effective date has not yet come and you want to cancel your plan by mail, skip to step 3.
If your effective date has passed, skip to step 5.

Call a Humana customer service representative and ask to have your insurance plan cancelled.

Write a letter to Humana asking to have your plan canceled.

Include your name, your Humana member ID number, the name of your plan, the plan's effective date, and your reason for canceling the plan.

Mail the letter to Humana.

Fill out the form and sign it electronically.

Click the Submit button to send your request.

Then all you have to do is wait for Humana's response. Do not proceed with further steps.

Print out the form.

Fill out the form and mail it to the address found in step 4.

Humana will notify you when it has processed your request.
