Use the School Website

The first step is to navigate to the Connections Academy website. Once you are there, click on the "Student Resources" tab and select "Transcripts." This will take you to a page that provides detailed instructions on how to request a transcript. Generally, you will need to provide your name, date of birth, social security number and the name of the school you attended. You will also need to specify the reason for your request. The online transcript request form is easy to complete and takes only a few minutes.

Visit the Connections Academy Office in Person
