Waiting more than 30 days from the date of purchase will most likely mean you cannot get one. To make a refund request, you need to contact Blizzard's customer service, and there are two ways of doing this.

Call Blizzard's customer support line. You will not have a chance to speak to an operator. Their phone support line only allows you to leave messages, so leave a message describing what product or service you want a refund for, and why.
Once you have made your request, Blizzard will decide whether to repair your damaged item, replace it with an equivalent item, give you a refund, or offer some other form of compensation. They may take a couple of weeks to come to their decision.
