The claim form inquires about a [redacted](k) distribution received in the last year and a half. I did receive a payment deposited into my old [redacted](k) by a former employer in [redacted] for the [redacted] match. Unfortunately, I didn't roll this over into my individual retirement account, resulting in a check with taxes deducted, as reflected in my [redacted] tax documents. Even though I received this distribution within the last 18 months, it's unrelated to my current employment since May [redacted]. This has caused a holdup in processing my claim for furlough time.

Hello, like many others, I am new to filing for unemployment. I was requested to provide a year's worth of information on gross income. Do I need to submit all pay stubs for this period, or just those from 6/30/19, 9/30/19, 12/31/[redacted], and 3/31/20 reflecting the income? I don't want to send unnecessary paperwork, considering everyone's busy schedules. My pay is received bi-weekly, so the income for 9/30/19 should be in my 10/6/19 paycheck, and the last December check was on 12/29/19. I also have my W2 form. Are these documents sufficient? What is the best information for you? I have income details from January [redacted] when I applied for Unemployment. I started the new job on January 13, [redacted], and was furloughed every other week due to COVID-19 from 4/6/[redacted]. How should I manage the bi-weekly scenario for unemployment? I was informed about my layoff on November 25, [redacted], and my last day of paid employment was on November 29, [redacted]. I had been employed from February [redacted] to November [redacted]. After finding out I was eligible, I tried to file a claim in January [redacted]. Initially, when asked for earning proof, there was difficulty identifying my employer. I spoke with Customer Service and mailed in my W2. I was informed that the steps were to provide proof, open the claim period, and then go back to December to receive assistance. Despite several attempts to resolve this, my claim is still pending. I have received two callbacks, but the personnel could not locate my claim. They mention working on COVID-19 applications. I am requesting a supervisor to contact me as I have the claim number. I need the determination corrected to open the period so I can apply from December 1, [redacted], or even January [redacted]. I stopped working on 3/18/[redacted] due to Covid-19. I applied for unemployment 4 weeks ago, but there's been no response on my claim. I've contacted my former employer who submitted the necessary paperwork weeks ago. Despite contacting the unemployment office seven times, I was told I'd receive a call back but haven't. I also submitted ten online call back requests with no response. My colleagues have already received payments, but I haven't due to incorrect dates on my claim. I submitted my unemployment application on March 22, [redacted], for the week of March 22-28. When I called on April 5 to report my work for the week ending April 4, the system asked if I was reporting for the previous week of March 22-28, and I accidentally said no, which caused me to get declined. I corrected it online but have not received payments for the weeks ending March 28 and April 4. I've only received two payments so far, and one is in process for the week ending April 25. I am wondering if I am entitled to two more payments. Also, when I applied on March 22, I listed "Temple Family Dentistry" as my employer. However, I was informed of my layoff on February 28 due to staff reductions. I only worked one day a week there. Although I did not include it initially in my application due to the pandemic, I submitted the details later as requested. I am uncertain if I will be considered for those earnings as well as I have not heard back.

I am currently unemployed due to the pandemic. I work part-time at Burlington Coat Factory in Natick, MA. I received one week of unemployment benefits because I was off work due to the virus. I declined benefits for the week of 5/5 as my employer provided me with 20 hours of sick time. I also declined benefits for the week of 5/12 as my employer gave me 11 hours of sick time. I now need to receive payments from unemployment starting from the week of 5/19 until I can return to work. I have not yet received the additional $[redacted]. I am unable to update my claim online to reflect 5/19. My account seems to be frozen. How can I resolve this issue and start receiving my much-needed unemployment payments promptly? I would like to discuss my claim with a representative. A few weeks ago, I had a conversation with a representative regarding my claim. They were supposed to close the existing claim, open a new one, and assist me as I tested positive for Covid-19 and could not work at the hospital as an x-ray technician. Although I received a form to end the previous claim, I have not received any updates on opening the new claim or submitting the required information. I have tried various phone numbers without success, filled out an online callback request form, and emailed a contact at the Unemployment office in Massachusetts. I am currently on day 39 of battling Covid-19 and trying to manage my stress levels to aid in my recovery. These additional steps are causing me stress and anxiety, draining my energy, and worsening my symptoms. I would appreciate a phone call or a direct number to speak with a representative. I spoke with a DUA employee on 5/1/[redacted] at 5:27 EST for 1 hour and 30 minutes. Nisha was very kind, but she was unable to assist me and suggested I talk to a supervisor in the morning due to the complexity of the issue. Since then, my claim details have changed, and when I tried to file for weekly benefits, it shows incorrect information, stating I have started a new job when I have not. I have been trying to apply for over 5 weeks and only received payment once. The information provided during the call with Nisha seems to be missing from my claim, causing significant financial stress as I cannot pay rent and bills. I filed an appeal on 4/13, also with no confirmation. The website states I should continue to request benefits, but I can't find instructions. I am seeking help with completing the 'Monetary Redetermination' form from Mass UI. I am currently unable to access a printer due to the lockdown restrictions imposed by the pandemic. I applied for benefits on 03/22/[redacted] and after waiting for weeks, I received a message stating I am ineligible, which surprised me. I was then told an email was sent, but I haven't received it. I submitted an appeal, but it's still pending. When I called for my weekly benefits, it prompted me to fill out a questionnaire that wasn't in my inbox. I believe I didn't do anything wrong while working for the company. I had an accident while driving the bus, which was unavoidable, and there were no serious damages or injuries. This incident shouldn't result in benefit denial, so I requested an appeal. Due to the current situation, finding work is difficult as people are avoiding public transportation. Hello, I am seeking assistance with my Pandemic Unemployment Assistance (PUA) issue. I recently received a notice indicating potential eligibility for Unemployment Insurance (UI) benefits, specifically for PUA, despite no changes in my circumstances. I have been receiving PUA benefits for 8 weeks as I do not qualify for regular unemployment benefits. The notice advises me to apply for regular unemployment benefits immediately, but also to continue requesting PUA benefits until a determination is made about my eligibility. I am looking for the mailing address and contact details for the Department of Unemployment Assistance so I can file a petition. They have been making it extremely difficult for me to receive my weekly benefits by not providing me with a PIN to log in and complete the process. Despite numerous attempts over the past month, I have been unsuccessful in obtaining the PIN. The process they have in place seems intentionally designed to prevent me from accessing my benefits. This includes asking for my SS number and then claiming they didn't receive it, redirecting me to closed offices, and telling me to call back during business hours. I am frustrated and feel like they are using unfair tactics to withhold my benefits.

Hello, I wanted to address an issue with my unemployment benefits. I have been receiving benefits since April 19, [redacted], even though I have been out of work since February [redacted]. On June 6, [redacted], I accidentally declined benefits, but there was no option to correct it, so I followed the instructions to reopen my claim. Hello, I began receiving unemployment benefits on 4/19/[redacted], which were expected to continue until 4/17/[redacted]. However, on 6/6/[redacted], I mistakenly declined benefits and was unable to rectify the error. After reopening the claim online the next week, I only received benefits for one week. I need to adjust the starting week for my claim to the prior week due to several job-related circumstances caused by the pandemic. Initially, in June, I had three part-time jobs. The Town Recreation Camp Councilor position was delayed by two weeks from the usual start date, resulting in no hours for the first week of 7/6-7/10. The following week, I only worked 4.5 hours there. Can I include this delay in my claim? Additionally, I left my MarketBasket/Demoulas job on 6/24 due to reduced hours and a change in job responsibilities that posed a risk to my high-risk family members. Lastly, my Nashoba Air job, which started in early June, has been consistent without any issues.

I have been enrolled with the Massachusetts Unemployment Department for the past three weeks. Yesterday, I encountered an issue when trying to enter the "telecert" number, as my SS# was not recognized. Despite signing in without any problems for the past two weeks, I got locked out after three failed attempts. Along with the login problem, I have also forgotten my password. When I seek help on www.mass.gov/dua, it seems that my SS# is unrecognized, leading to continual blocks. I have been unsuccessful in reaching anyone to resolve these issues.
