16 digit card number, Name on the account, Last 4 of SSN, Billing Address, and Your birthday
The issue in GetHuman-fourcar's own words
Credit card statements and late pay alerts were sent to old Email address that I don't monitor. First time used card in over a year and contact information was incorrect. In addition, my account was changed to on line without my authorization. Late payment information was sent to my credit reporters and has resulted in an excellent score dropping by ** points. I have repeatedly sent messages to sears and got canned responeses about the virus.

GetHuman-fourcar did not yet indicate what Sears Credit Card should do to make this right.
