On August 2, [redacted], I placed an order for a birth certificate for Gary S.D. Stone, costing $47.00, with the expectation of next morning delivery to the County Health department. The Health Department informed me they don't accept overnight deliveries. The order was made via Gary's email, [redacted] Gary, in Jacksonville, FL for school registration, obtained the certificate locally the same day. I was charged $47.00 on my credit card ending in [redacted]. I am Mary Sue H. of [redacted] N.W. Brady Circle, Lake City, Florida, [redacted]. Kindly refund the $47.00 to my credit card.

I mistakenly ordered a birth certificate through your agency expecting the long form but received the short form twice after paying an extra $40 both times. I'm disappointed in the misinformation provided by your employees. I contacted the New York City Health Department and was given the correct instructions to obtain the long form. Now I am out $80 when it should have only cost $40. I believe it is your agency's responsibility to rectify this issue as it was the result of inadequate information provided by your staff. I am requesting a refund for the money spent on a long form birth certificate that I did not receive. Thank you.

I recently ordered a birth certificate from your agency, expecting the long form but received the short form instead. When I inquired about obtaining a certified copy of the long form, I was advised to choose Passport as the reason for the request, resulting in an additional charge of $40. Unfortunately, I received another short form. I am disappointed with the service provided by your agency. After speaking with the New York City Health Department, I was informed of the correct procedure to request the long form. I now find myself out of pocket by $80 when it should have only been $40. This issue rests with VitalChek, not NYC, due to misguidance from your staff. Please do not instruct me to contact NYC to resolve this matter as it falls under your responsibility to provide accurate information.

The email address used for correspondence was [redacted] Following your instructions in an email, Gary went to the Columbia County health department in Lake City, FL to pick up the certificate the next morning. However, they informed us that they do not accept overnight certificates and only handle certificates issued on-site. As we were unable to receive the certificate from your end, we kindly request a refund of the $47 to my credit card. I recently visited the health department where my son was born and discovered they had my last name incorrect on his birth certificate. When I called to address the issue, the manager threatened me legally and asked me to stop calling. Feeling frustrated, I told the person who answered that, due to my disability, I could have her fired for this. I am tired of arguing with them and the Indiana State Health Department. I would like to remove the father from the birth certificate, as he also wishes to be taken off. This has been an ongoing issue since April 12, [redacted], and despite finally receiving the birth certificate, the corrections have not been made as they should. It is important to me that the stepfather's name is removed as my son does not want him listed.

I purchased a birth certificate from VitalCheck in January [redacted], but never received it. After contacting customer service, I was promised a refund. However, the next day I was informed that my order had been shipped. Confusion arose when they mentioned the refund would come after the package was sent. Despite clarifying that my account was closed, they inquired about my bank details. Frustrated by the back and forth, I provided my address for a check refund. More emails followed asking about my bank, leading to further exasperation. I expressed my frustration at the inconvenience and requested clarification without receiving a satisfactory response. As of now, I am still waiting for my refund and hoping to resolve this issue promptly.

I placed an order for a certified birth certificate for my mother, Jenny F., on September 10th. The original documents were sent out on September 11th to the NYC Department of Health and Mental Hygiene following the instructions provided. However, I am receiving repeated notifications stating that my order might be canceled. After contacting VitalChek, they directed me to the NYC Department of Health and Mental Hygiene, who claim they have no record of my order. In the meantime, I am without the original documents.

Despite my repeated inquiries about its status, I consistently receive the same automated response indicating delays due to a high volume of requests from New York. I have been attempting to obtain my birth certificate through your company since August. Despite submitting the requested information twice and having the payment deducted from my account twice, I have not yet received the birth certificate. I eventually resorted to obtaining it directly from the Indiana courthouse. I am seeking a refund and would appreciate guidance on the process. I hope the refund procedure is more efficient than acquiring a birth certificate through your services.

Regarding Order [redacted] -- Urgent I have been waiting for an order I placed two weeks ago today for my divorce decree from Hood River County, OR. I paid $63.25 for this order. The status on your website has been "processing order" until today, when it suddenly changed to indicate that more information is required. I need this document for my social security, which requires it within 15 days - that deadline is tomorrow. Can you please assist me with this issue? Thank you.

I need to obtain a certified copy of my daughter's birth certificate. I recently applied for a passport, but it was rejected because I mistakenly submitted an abstract instead of the birth certificate. I remember seeing an option to have the certified copy sent directly to the passport office. How can I find that option again? I want to expedite this process as our trip is scheduled for the 27th.

I completed the VitalChek online form for a birth certificate from the District of Columbia to obtain my Real ID. When I reached the payment section, I hesitated to enter my credit card details as the application did not request any identification documents, unlike the hard copy form from the DC government. I am uncertain about the process and whether I will receive my certified birth certificate through the mail, which the DMV will require for my Real ID application.

I submitted a request for my birth certificate nearly three weeks ago. I was born in Newport News, VA. I received the packet, completed the form, and returned it. However, I have not received any confirmation that my order is being processed or has arrived. I am also missing the email containing my pin and code confirmation. This is my second attempt to obtain my birth certificate, as I faced the same issue during my previous try. I urgently need my birth certificate to apply for a Real ID for my Nevada driver's license. I am inquiring about obtaining a copy of my birth certificate. I have included my marriage certificate, divorce papers, and both my social security cards (one with my maiden name and the other with my married name, which is still my legal name). How can I provide evidence of my last name change that occurred when I married nearly 40 years ago? After my divorce, I retained my married name and did not revert to my maiden name. The marriage and divorce documents, along with the two social security cards displaying both names, should suffice. Who should I contact to provide the requested documentation? My last name changed when my husband, who was in the Army, and I were in Germany over 40 years ago. At that time, I had to use his last name for my military ID and tax purposes without additional paperwork. What should be our next course of action?

Hello, good morning. My name is Braskara Adams. I was born in the United States in Manhattan, New York, but I have been residing in the Dominican Republic since June [redacted]. I currently need to obtain an updated birth certificate because the one I have is a bit old. For the process of adjusting my status, I went to the United States Embassy in the Dominican Republic to obtain a passport or some form of identification. My question is, how can I get a certificate if the requirements ask for a social security number, and I do not have that information? Since I have never been to the United States after coming to the Dominican Republic, I do not know my social security number.

On January 8, [redacted], Barbara from Vital Check cancelled my order [redacted]45 with PIN [redacted] for a marriage certificate. On February 4, I contacted them online and provided reference # [redacted]80 to correct the marriage date to January 8, [redacted], not January 9 as originally submitted. This event occurred in Escondido, CA, San Diego County. After a 1 1/2 hour wait on the phone, the agent fixed the date to Jan 8, [redacted]. Following their instructions, I redid the notary on February 7 and faxed the document to VitalChek. Surprisingly, on February 8, Barbara emailed me indicating the order was canceled. I explained my situation but received no response. As I reside in Hawaii, I rely on online services and facing challenges with their process. If I have to restart, I am concerned about the errors that may arise. Your help is appreciated.

I ordered my birth certificate from NY on 1/7/20, but living in VA is causing delays. The processing time has increased to 35-40 days from 15-20. Today is 2/18/20, and I haven't received any shipping notification. I need the certificate to pursue a new job opportunity. Despite paying for faster shipping, I cannot track my order online, and the phone line is unresponsive. The delay is hindering my career growth as my current work environment is challenging. I have an email receipt from VitalChek dated January 31, [redacted], confirming my order of 3 copies of my certified birth certificate. On the same day, I also received an email from the NYC Department of Health stating that my order has been completed and providing a UPS Tracking Number: 1Z038Y[redacted]69. They advised waiting 10 business days before tracking the order and not to reply to their email. However, it has been well over 10 days and I have yet to receive my package. On January 25th, I was notified that my application was successfully received. I was given Order Number 9cb3fce3 and informed that an agent would review it for accuracy. On January 26th, I received an email confirming that my application was compliant, so I promptly filled out the necessary forms. The next day, January 27th, I followed instructions to send the completed documents, a copy of my driver's license, and a $10 check to the Department of Health in Indianapolis via certified mail. The package was received on January 29th, signed for by Larry W. Manning. I have not yet received the birth certificate, even though my check was cashed on February 26th. The confirmation stated that the birth certificate would arrive in approximately 3 weeks, but it has been 7 weeks. I am eager to know when I can expect to receive the birth certificate.
