I am currently trying to obtain my father's birth certificate from American Samoa. I completed the in-person process with the Samoan office in Honolulu, HI and was informed that it would take approximately 3-4 weeks. This was back in March [redacted]. It is now November 24, [redacted], and I have not received any updates regarding my father's birth certificate. Despite following up every Friday after the initial 3-week period, I have not received any word or assistance in obtaining the birth certificate. I have also tried contacting them via phone, but no one answers and the voicemail box is either full or unavailable. I visited the office multiple times, and at no point was I informed of any additional documents needed for the process. I have also attempted using the VitalChek online service, but due to my father being homeless and his documents being stolen, I only have copies of his expired HI license and American Samoa birth certificate. These documents were not sufficient to renew his ID. The original of his Social Security card, which is currently being processed, is the only original document I have left. I reside in Nevada but have temporarily left my job to care for my ill parents. I have exhausted all possible options and am seeking guidance on how to proceed.

I placed an order on October 26, [redacted] for my father's death certificate (Order No. [redacted]8), with an expected processing time of 15-20 days. However, I have yet to receive any updates from the NYC Dept. of Health, and the UPS tracking number provided shows no movement. Despite multiple attempts to contact the department of health, I have not been able to obtain any information about my order. This delay is frustrating as I urgently require the death certificate. I had hoped to avoid visiting the Dept. of Health, but it seems inevitable given the lack of progress. I would appreciate clarification on your relationship with the New York City department of health and a resolution to this issue. Thank you.

I ordered a birth certificate from you on March 21, [redacted]. I reside in France and the delivery fee was $57.80. I actually require a long-form birth certificate with a letter of exemplification and it needs to be apostilled. I will be in the US for two months and will then return to France in mid-February. Could you please forward the form to the New York State Department for the apostille, who can then send it to my sister's address in Florida? This method should be more cost-effective than mailing it to France. Is there a way for me to email a scan of the birth certificate you provided? The order number is [redacted]1. My date of birth is February 15, [redacted], and I was born at St. Clare's Hospital in Manhattan. My name is V.S. Gerber.

I am inquiring about the status of my UPS/FedEx order. The automated system states that the document was sent to UPS on 01/22, but the UPS tracking system only shows that a label was created by the shipper and not the actual package for delivery. I urgently need this certificate, and I am concerned it may not have reached UPS despite the system indicating it did. Can you please assist me in locating the document and ensure it is sent promptly?

I need assistance regarding a lost birth certificate handled by UPS. They are requesting a claim for a replacement. My order number is [redacted]9 with the pin [redacted]. I can't be reached at my job number [redacted], so please provide an alternative. The current customer service wait time is extensive, and I am unable to leave work to receive the replacement in person. Could the birth certificate be sent via regular mail? I am currently in Ohio without a picture ID due to losing my Texas DL. Urgent help is greatly appreciated.

I have been waiting on hold since 10:30 am this morning trying to reach a representative without any success. It has been two hours now. I am frustrated that a company handling vital documents can subject customers to such long wait times. Is this level of service acceptable? I recommend avoiding Vital Chek and suggest visiting the Department of Health in person instead for a smoother experience.

I have received my order for a birth certificate from Indiana, but the document inside the envelope is a certificate of birth instead of a birth certificate as expected. I was under the impression that I needed the full "birth certificate" for passport purposes and need assistance on how to proceed with obtaining the correct one. Can the existing order information be used to correct this error, or will I need to place a new order? This situation has caused me a lot of trouble, and I would appreciate some assistance with this matter. Thank you. - Ranse Eugene Weatherford

In October [redacted], my father passed away. I used Vitalchek services to obtain his death certificate to claim the life insurance he left me. Payment was made with a Visa card. Despite numerous emails sent monthly, providing the same details each time, I have yet to receive the certificate. I reside in North Carolina, four hours away, and have submitted a copy of my photo ID. The necessary death certificate is crucial for me. My name is Tammy Handy, and I can be reached at [redacted] Rollingwood Drive, Hays, NC [redacted]. I am 58 years old, reachable at [redacted]. The deceased is Joe Wayne Anderson of [redacted] Texaco Town Road, Melfa, Virginia, who passed away on the 13th or 14th of October [redacted]. His wife’s name is Shirley Anderson. All communication with Vitalchek has been documented for reference.

I am reaching out concerning my request for a birth certificate with order number: [redacted]2 and pen number: [redacted]. Despite being 73 years old, I am not very tech-savvy and encountered an issue while trying to provide a clear copy of my current driver's license. When I check my status on your website, it shows "documentation being reviewed," but I have sent the required copy multiple times without success. Time is of the essence, especially since the GA offices I need to visit will be closed over Memorial Day weekend, restricting my window to submit the birth certificate. I am anxiously awaiting confirmation that my latest attempt to send my driver's license copy was successful. Thank you very much for your assistance in resolving this matter promptly.

I recently received a letter stating that there is no record of my daughter's birth certificate. This is quite concerning as she was born in Riverside County and there should be a record available. The letter only mentioned a refund of the shipping fee of $19.00, but I paid a total of $59.95 and did not receive the documents I requested. This situation is causing me inconvenience as I need the birth certificate to enroll my daughter in school. I am planning to visit the county clerk's office tomorrow, which is a 50-mile trip for me. Any guidance on how to proceed with obtaining a full refund would be greatly appreciated.

Using VitalChek to obtain my marriage certificate was a frustrating experience. After entering my credit card information and receiving a confusing email a week later about a missing form, I struggled to navigate their website without clear instructions. Despite filling out the form from the California clerk's office, getting it notarized, and faxing it multiple times, I faced challenges with the notary's seal. Communication issues about fax problems further delayed the process. Now, I have to wait longer for their quality control team to approve the official form, prolonging the already lengthy processing time. If we had known about the complications involved, we would have chosen to handle the request directly through the local government office instead of using VitalChek. I recommend avoiding their service based on our frustrating experience.

I recently spoke with the NYC Department of Health, and they directed me to reach out to your office regarding my Birth Certificate application. I received an email stating that the order was voided due to an error in my mother's first name on the application.
Details:
Order No.: [redacted]8
PIN: [redacted]
I am uncertain about the name I provided for my mother on the application and need to verify before submitting a new one. My mother was known as "Beverly," but her legal name might be "Bertha." Can you confirm if "Bertha" is the correct name to use? Additionally, I believe I listed my father's name as "Jacob" although he was called "Jack."
Both of my parents have passed away. Please clarify if I should proceed with submitting a new application for processing.
Thank you,
Terry Mark Strassberg

My name is Joan Josephine Saccio. It's on my social security card, birth certificate (which I misplaced), and my expired passport. I ordered a new birth certificate to match my expired passport, but the one I received only shows "Joan Saccio" and not my full legal name. I've been using my full legal name for over six decades and need it to match my expired passport for my new one. Please send me a corrected birth certificate with my complete legal name as soon as possible. I need it by next week. Thank you, Joan Josephine Saccio.

Hello, I found the process with VitalChek frustrating. I followed their ID upload instructions yesterday. Now they require me to provide my mother's maiden name by calling 1.[redacted]. However, when I called, all I encountered were messages notifying me of long delays due to high call volumes, suggesting I call back later.
The maiden name they are requesting is 'GAZZOLA'. Can you assist in getting this information to them?
Thank you,

I placed an order for my son's birth certificate on the second. I initially emailed a confirmation and had to resend it on the 5th. Despite encountering a similar issue in May with my own birth certificate order, I have not received any communication from your team. I urgently require my son's birth certificate for his school enrollment. It's frustrating to anticipate another 10 business days for its delivery when I've fulfilled all requirements promptly. This marks the second time I've faced this issue with your service. Although you mentioned a refund, I fail to see the necessity as I've provided all the required information and made the payment. I hope you can swiftly address this matter and dispatch my son's birth certificate without necessitating a refund. Your prompt assistance in resolving this would be greatly appreciated.

I ordered my birth certificate and patiently waited for its arrival, but it never showed up. This was a new and frustrating experience for me. Their delay has affected my work prospects, and despite eventually offering a refund, the overall process was disappointing. They asked me to complete a form for additional fees to expedite the birth certificate, only to face more waiting. Sadly, their lack of reliability and concern has left me without the vital document I needed. Now, I find myself waiting for the refund, a process that seems unnecessarily prolonged considering how quickly they debited my bank account. This ordeal contrasts sharply with previous successful attempts to obtain my birth certificate. It is advisable to avoid using their services as they have repeatedly faltered in their responsibilities.

I placed an order through VitalCheck for a certified copy of a marriage certificate from Cook County Genealogy on July 26. However, I am unable to obtain any updates on the status of my order. Despite my efforts, neither Cook County nor VitalCheck can locate it. After spending a considerable amount of time online and on the phone, I was informed by someone at VitalCheck this morning that the order was mistakenly sent to the Illinois Dept of Public Health. Upon contacting them, I discovered they do not process online orders from VitalCheck for marriage certificates. They only handle marriage verifications through written applications and ID submission. I am at a loss and no one seems to have information about my order. I am hoping a supervisor from VitalCheck can contact me to investigate the whereabouts of my order.

I recently attempted to order my birth certificate under the name Bonita Jane Grimes from the Indiana State Dept. of Health using VitalChek on May 14, [redacted]. My Order Number is [redacted]5 and Pin is [redacted]. Despite paying $21.00 for UPS Air shipping, I have not received my birth certificate. This delay poses an issue as I need it to renew my State ID promptly for an upcoming trip to adopt my new seeing eye dog.
Given my limited income, the total cost of $38.50 is particularly burdensome, and I am requesting a full refund. While I was informed that a refund was issued to my Mastercard, I have not yet received confirmation from my credit card company regarding the processing of the refund.
Despite setting up a refund request via phone and sending a follow-up letter, I have not witnessed any progress. Your attention to this matter would be greatly appreciated.
Sincerely,
Elizabeth A.
