I need help with my birth certificate order from Arizona Vital Records. The UPS couldn't get my signature, so the order is going back to them (Order Number [redacted]4, PIN [redacted]). They expect to receive it on 3-31-16. I need the birth certificate urgently for my passport. My address is correct, but I was away. I'm back now and willing to pay extra for quick delivery. How can I ensure the birth certificate is sent back to me promptly? Please provide guidance on the necessary steps. Thank you, Jonathan P. Dovin

I have yet to receive the Missouri birth certificates that were ordered on 04-04-18 for Robert S. (09YINTXV) and Broderick S. (R9Z3MWQG). I encountered an issue when ordering Robert's certificate as the system locked up. After waiting and restarting my computer, I resubmitted the order for Robert while also placing an order for Broderick. Following this, I spoke with my credit card company the next day and learned that I was being charged for three certificates. Quickly, I reached out via email to cancel the extra orders but have not received a response. Despite attempts to contact your customer service via phone, I faced long hold times and disconnections. I am currently without certificates, with unanswered emails, and charged extra. I seek assistance in resolving this matter promptly. Thank you for your help.

I ordered a divorce certificate, and it arrived quickly, but with the incorrect name of my ex-husband. After contacting VitalChek, they directed me to reach out to the court. The court confirmed the correct name, indicating VitalChek's error. I have requested a refund from VitalChek but have not received a response yet. I demand a refund as the certificate is invalid for legal purposes. 😒

Hello, my name is Darryl B. My order number is [redacted]2, and the pin number is [redacted]. I requested a copy of my birth certificate on April 9th. I have submitted all the documents requested, although I had to resend them multiple times. I was charged twice for the copy but have not received it. An issue cited by the New York Department of Health Vital Records Section Certificate Unit mentions payment problems. If more money is required, I would like to be informed. I am awaiting a resolution and appreciate anyone who can contact me to clarify the next steps to obtain my request. I have been waiting by the phone for assistance without success. Kindly provide guidance on how to complete this process.

I need help with obtaining my birth certificate. My last name is now Benfield, as I was adopted when I was around 9 or 10 years old, but my original last name was Green. I started the online application process by entering details about names, addresses, and parents. However, I encountered an issue when I had to provide my signature with my legal name, which differs from the name on my birth certificate. This caused me to be unable to proceed with the order placement. Can you advise me on how to resolve this so I can expedite the process within a week? Thank you.

I would like to request State Vital Record Certified True Copies of Birth Certificates (Long Form) for my son and daughter. When I entered their details on the website, it directed me to order from the Yuba County Recorder's Office in Marysville, CA. However, upon contacting the office, I was informed that they do not have the Long Form State Vital Record Copy available. I am in urgent need of these documents and would like to know where else I can obtain them. I am hoping to collect the Birth Certificates in person if possible. Your website only lists the Yuba County Recorder's Office as an option, so I am seeking guidance on alternative locations to acquire these vital records. Any assistance you can provide would be greatly appreciated. Thank you.

Hello!
I am interested in obtaining the birth certificates for my granddaughters to process their dual nationality. They were born in San Diego, California, and currently hold American nationality. However, both their parents are Mexican. The girls now live with me and their mother in Mexico, which is why I am looking to get their birth certificates apostilled and certified if possible. Can this be done via VitalChek? As their maternal grandmother, I would like to know the duration of the process and if the certificates can be sent to Mexico. Regarding payment, is it possible to pay with cash or only with a debit or credit card? The card belongs to my niece; can she make the payment with a credit card? Your assistance in this matter is greatly appreciated.
Thank you,
Margarita Argueta Castro

I am reaching out regarding order number [redacted]2, which has been canceled due to incorrect information related to my biological parents. My birth mother, Vanessa Burks, and father, Manuel Francis, passed away when I was three years old. My adopted mother, Helen Rose McNeely, who raised me, was never married, and she legally changed my name from Manuel Ernest Francis Jr. to Manuel Ernest McNeely Jr. after she adopted me. I was born on January 17, [redacted], at Long Island College Hospital. Unfortunately, I have limited access to my birth certificate and only saw it once at the age of 11. Helen Rose McNeely has recently passed away, and I am unaware of any siblings I may have. She adopted me through an agency called Gram Wyndham. I have provided all the information I have as I am in the process of obtaining my commercial diving license. Your assistance in resolving this issue is greatly appreciated. Thank you.

I am having trouble getting my marriage certificate from VitalChek. I've been trying for two days online with no luck. I completed the form online, but I wanted to rush the order, and I couldn't find an option to do so after providing my payment information. This is urgent as I won't be able to drive or title my car because my driver's license and car tags expire at the end of this month on August 31st. The request has already been sent from VitalChek to the state office in Florida, Jacksonville, but I can't reach them. I really need this certificate to take care of my car and driver's license situation.

I paid $70.30 to VitalChek for three copies of my birth certificate. The order was accepted on 8/24/[redacted] but then rejected on 8/27/[redacted] because the email address didn't match the name on the request. I used my daughter's email since I don't have one - I'm 95 years old and not tech-savvy. I correctly answered all security questions on the form. VitalChek only refunded me $60 blaming my bank. Where's the $10.30 for the undelivered service? They shouldn't charge for something they didn't do. Despite creating an email for me, I'll mail in a request for $45, with a notarized signature, and wait the 8-10 weeks. I've lost faith in VitalChek for a full refund.

I recently ordered a birth certificate for my son to be delivered to Bryam, MS, although I live in Woodville, TX. Due to UPS policy in Jackson, MS, I can't pick up the birth certificate myself. Despite providing information to UPS about who would pick it up, they attempted delivery without me present. Each time I called UPS, I was told that anyone with a government ID showing the same address could collect the package. The local Jackson store informed me this wasn't their policy, contradicting what the customer service representative, Loretta, had told me over the phone. Loretta was quite rude and mentioned the package would be sent back to VitalChek. The situation has left me feeling frustrated, and I am urgently requesting a call back to resolve this matter.

I have been waiting for my birth certificate since August. I ordered it through Vitalchek. They sent me an email on September 4 with the tracking number and invoice showing they deducted $57.95 from my bank account. The order number is [redacted]0 and the tracking number is 1Z011Y[redacted]11. Unfortunately, UPS has not received the package yet. I was initially told it would arrive in 7-10 days after payment, but later they said 60 days due to Texas regulations, although a tracking number was provided. I urgently need my birth certificate and can't afford to be scammed. If I don't receive my certificate or a refund from Vitalchek soon, I will report this to the Better Business Bureau and the FBI.

I am reaching out from Danjolell Memorial Home in Broomall, PA regarding three orders that require proper ID and credit card information. I have provided driver's licenses, but they show a home address while the Death Certificates are to be mailed to the Funeral Home. The issue is that without the names of the deceased requested, I cannot address this problem. I only have the order numbers and Pin numbers at my disposal. I am aware of privacy concerns but unable to proceed without the necessary information. The order numbers are as follows: [redacted]4 Pin [redacted], [redacted]9 Pin [redacted], and [redacted]6 Pin [redacted]. I have attempted to call but faced long wait times. Assistance with this matter is greatly appreciated. Please contact me via email for any additional details. Thank you. - B. Kinard

I uploaded all the necessary documentation yesterday. Each PDF file has been uploaded four times for clarity. I am a disabled person, so phone calls, fax machines, or long computer sessions can trigger seizures. Despite this, I managed to complete everything yesterday. I received an email this morning at 5:00 a.m. stating that I had 7 days to upload the documents or my order would be canceled and I would still be charged. This does not seem fair as I have uploaded the documents multiple times as requested within the 24 to 48 hours timeframe. I do not wish to be charged a fee when I have complied more than once. My name is Darlene A., and my phone number is [redacted].

I recently had an unpleasant experience with your customer service representative, Leslie, during a phone call to cancel an order. Leslie's tone was condescending, and she did not allow me to fully explain my situation. She mentioned my documents were missing and that the order was incomplete, but instead of offering guidance, she directed me to contact another office. I found this frustrating as I believed I was contacting the correct company, VitalChek, to cancel my order. Despite being on hold for over 30 minutes, I maintained a polite manner. When I sought clarification on the phone number and office hours, Leslie's responses were abrupt and unhelpful. She assumed I was familiar with the cancellation process without allowing me to ask questions. Overall, Leslie's handling of the call left much to be desired in terms of customer service satisfaction.

I am requesting a full refund for my brother's death certificate ordered on October 24th. However, I have yet to receive it, and I encountered issues concerning the $40 processing fee. While I comprehend the $8.30 search fee, I am unwilling to pay $15 for an undelivered certificate and $17 for its shipping. This situation is unacceptable, and I am determined to pursue a refund. I may look into uniting dissatisfied customers to address this matter further.
