Account holder name, Billing address, Phone number on account, and Last 4 digits of SSN

My name is Pamela Woodfin ordered SIM on card ***. Rec'd notice that if I wanted * continue service or chg plan. I chrgd plan to the free plan. On **** was chrgd $** *** $**.** *** $**.** *** .**, *** chrgd $**.** & **** $**.** billed twice. Phone has never had dial tone where bars are it had an R So I removed SIM card. Thought that was the end of it. So what I'm saying is I need all chrgd refunded
