Please do not call my cell phone or work number as I am not allowed to take calls at work. You are also advised that you will be fined up to $**** for each offense.*I am revoking authorization and requesting that you cease all automatic payments from my bank account and*or debit card. You are no longer authorized to deduct payments of any denomination from any bank account and*or debit card I have on file with you.*Additionally, I no longer need the above-referenced account and ask that it be closed immediately.
