My husband has become critically ill, his EDD disability claim was approved ********* and we were informed to watch for a Edd Card. However as of this date, we have not received his Edd Card and need the funds as he will be coming home shortly for Hospice Care.**Speaking of end of life care, what forms does the EDD need for me to fill out in order to act as his power of attorney on his account?**Also I do have a power of attorney that I could send to some one and it is Update and Notarized.**As it is now, I cannot reach anyone and naturally no one will assist me without the proper EDD forms*Thank you CARemington
