I am currently in the process of filling out the lost passport form after my husband misplaced my document. I am unsure whether I should select the option that asks, "Are you submitting this form in connection with an application for a new US Passport?" I am obtaining a replacement for my current passport that I believe is valid until [redacted]. Should I proceed to fill out an application for a passport renewal since the previous one is lost? I recently received a letter requesting a complete court order with a judge's signature for my passport name change. The legal paperwork for the name change and the $[redacted] fee were sent with my old passport, as instructed. Unfortunately, I do not have a complete court order as part of the name change process. I am confused about what additional documentation is needed to proceed with the name change on my passport. Hello, I recently found out that I need to travel to Germany for work next month. Unfortunately, I realized my passport has expired, and I can't seem to locate it in my home. Additionally, I am unable to find my birth certificate, but I do have a certificate from the Air Force base where I was born. This certificate includes details like the US Air Force hospital name, my name, my parents' names, date of birth, and a signature. It also has a hospital register number, but it is not notarized. I'm curious if this document would be sufficient for secondary verification purposes.

We have a group of 10 set to embark on a cruise tomorrow. Unfortunately, my brother-in-law fell victim to a pickpocket who took his passport. He needs it to board the cruise on August 12, [redacted]. He has already obtained a police report from Venice, Italy. His contingency plan is to have one of his children mail his expired passport alongside his current passport card. Despite this, we are unsure if this will suffice for boarding tomorrow. We are hopeful if there is a way to expedite a replacement passport to the ship to salvage our long-planned trip. I am unsure whether I can renew my passport online or if an in-person or mail application is necessary. Additionally, I am unsure about the correct form required for the renewal process. There has been a name change from my previous passport, as my driver's license now displays my accurate birth name instead of the nickname. This change was only reflected in my driver's license, and there is no official documentation like a court order to validate the name alteration.

I sent my passport application to the correct service center on 17 July [redacted], the payment check cleared on 20 July [redacted], but as of 16 August [redacted], the online status check does not show any updates. When I called the passport services, the voicemail system did not offer an option to confirm if my application was being processed. Proper systems should acknowledge receipt upon arrival and especially when payment is received. I believe a resolution should include a confirmation from a passport services employee verifying the application's receipt and details.

I recently received a letter about my passport application. I already have a valid passport and just want to change my name back to my maiden name. I submitted the certified Divorce Decree from the Courthouse along with proof of my Drivers License. However, the Passport agency sent me a letter requesting a certified copy of the court order or marriage certificate reflecting my legal name change. My divorce should serve as proof of my name change. How should I proceed since I am unable to reach a live representative to discuss this matter with them?

I submitted my passport renewal application at the Raley Post Office in Rohnert Park, California on August 1st. I followed all the necessary steps, including providing my old passport, the completed application form, passport photo, and the renewal fee. However, I was surprised to receive a letter on September 26th claiming that I had not included my old passport, even though I had sent it with my initial application. I am now awaiting further instructions on how to resolve this issue.
