Subject: Reconsideration Request for Insurance Claim Denial
To Whom It May Concern,
I am writing to appeal the denial of my coverage claim. On March 5, [redacted], I contacted Meritain Health to seek preapproval for a Colon Screening test as per my doctor's recommendation. I specifically inquired about the Colon Guard Test, which is a colon cancer screening conducted at home. I spoke with Maryland B. who assured me the test was In-Network and [redacted]% covered as a preventative measure, leading me to proceed with the test. However, I later received a denial from Meritain on 5/15/[redacted], stating the test was not covered.
Upon discussing this issue with Meritain representatives, I was given conflicting information. I was informed that the test was out of network and that there were form inaccuracies that needed correction. I felt misled by the initial information provided on March 5th and the subsequent clarification.
I urge you to review the recorded conversations I had with Meritain on 6/4/[redacted] and 6/5/[redacted]. I trusted the information I received before proceeding with the screening, and I am now facing unexpected financial implications due to this discrepancy.
