What are the requirements for one to join the National Diabetic Prevention Program? Can you be enrolled in two programs simultaneously? For instance, I am currently in a program with monthly meetings, and it has been really successful for me. I am interested in starting another program that has weekly meetings while continuing with the first program that meets once a month for six months. I do not have diabetes, but I am morbidly obese, and my mother had been insulin-dependent for 40 years, suffered from 5 heart attacks, and lost a leg. Additionally, I have an aunt with diabetes. Your assistance in this matter would be greatly appreciated. Thank you.

My name is Nicky Thomas, and I work for JF&CS. I have previously raised concerns with the CDC regarding a coworker who had open sores. Despite his return to work, his sores are not fully healed. We have observed him coughing over clients' food, not washing his hands, and improperly disposing of bloody bandages. There are also hygiene issues in the bathroom. He has a history of pneumonia and is now coughing without covering his mouth, which is concerning. Several staff members, including myself, fear that his sores may have returned and are leaking. Please assist us in addressing this urgent matter.

I perform ablutions five times a day as a Muslim using icy water. I live with diabetes insipidus and consume 3 to 5 liters of water daily. Eating and drinking adequately is crucial for my well-being. My mother, a retired cook, supports me financially as I am in the disabled 2 group. Despite being ill with the flu recently, I didn't seek treatment and managed the symptoms for three days. I get eight hours of sleep daily. My strong immunity is evident in my lymphocyte count, exceeding the norm even after being sick. Living with a rare degenerative disease for 25 years, my immunity has been crucial in slowing down its progression. Though there's an expensive experimental treatment involving stem cells, it's beyond my means. My condition, diabetes insipidus, has exceeded its life expectancy, with remarkable kidney function and heart health despite the challenges I face due to excessive water consumption. My unique immunity offers hope for innovative influenza treatments.

I am a Muslim who performs ablution in icy water five times a day. I have diabetes insipidus and drink 3 to 5 liters of water daily. Despite my health conditions, I lead a healthy lifestyle, supported by my mother who is a retired cook. I am in the second disability group since childhood and sleep for 8 hours. Recently, I had a flu with symptoms lasting 3 days. I did not treat it, and my immunity, shown through lymphocytosis in multiple tests, helped me recover. I also have olivopontocebribular degeneration but manage it well due to my strong immunity. Though my medical conditions are challenging, I continue to live with hope. I have explored experimental treatments like stem cell therapy for my degeneration but find them costly. Despite the odds, I am grateful for my health and the support of my loved ones.

I am currently residing in a temporary accommodation where a woman with a severe parasite infestation, including scabies, resides. This has been a concern for me because of recent cataract surgery, making me more susceptible to infections. I fear for my health and safety due to the possible contagious nature of her condition. There are parasites visible on the floor and in the food, raising alarm. I do not feel safe sharing the same living space with her, especially since she has no official lease here. I kindly request assistance in having her removed and properly tested to prevent any further spread of parasites. If there are any steps I can take to address this issue promptly, please let me know. Thank you for your attention to this matter.
Best regards,
Julie F.

As a chronic pain patient, I was in an automobile accident in May 9, [redacted]. I have been on opioid pain medication for over 20 years, but recently they have reduced my oral medication by half and completely removed the fentanyl patch. Even though the Fentanyl box specifies it is for opioid-tolerant patients, my pain levels are increasing, and my blood pressure is spiking. My Pain Doctor says there are no immediate alternatives. My pain has worsened, keeping me up at night and causing severe suffering. I was on 100mcg of fentanyl and 150mg of oxycodone daily. I fear a heart attack or stroke may occur due to inadequate pain management. Long-term chronic pain patients, like me, are suffering greatly. I hope the CDC can consider exceptions to their rules to ease the distress of patients and relieve the fear doctors face in prescribing necessary medications. Your attention to this matter would be greatly appreciated. Thank you, KP.

I reside in an assisted living facility in Sandy Springs. Recently, I encountered a concerning issue with the air conditioning unit in my room, as it was filled with mold. Despite my efforts to clean out a significant amount of black mold, I suspect there is more left. The environment in the room while cleaning was toxic to my eyes, nose, ears, and throat due to the mold. I requested assistance but received minimal help. The room needs to be thoroughly cleaned to ensure it is safe for me to return. I am experiencing a severe migraine which I believe is related to the mold exposure. Please contact me via phone at [redacted] as I do not use email frequently. Your prompt response is greatly appreciated. Sincerely, Cheryl.

I live with my family of four at Botford Terrace Place Apartments in Farmington Hills, MI, and faced a severe issue 15 days ago. While I was taking a morning shower, the bathroom floor collapsed due to a broken pipe, causing damage to both the bathroom and bedroom. Despite reporting the problem, it took 12 days before any action was taken. When I spoke with Barbara in the office last Friday, she did not provide satisfactory assistance regarding the mold issue. Over the weekend, my family and I discovered more water damage in the crawl space, leading to concerns about our health and safety. The unbearable smell, mold growth, and my deteriorating health have forced us to sleep in the family room. This situation has taken a toll on us, with my husband's behavior drastically changing and causing distress. I urgently seek guidance on how to address this serious problem with the apartment management.

Dear CDC,
I tried reaching out to the city of Lincoln, Nebraska regarding drainage issues, but have not received a response. I have had pneumonia twice since West Nile and am now experiencing tremors in my hands among other symptoms. I believe the city is responsible for maintaining curbs and street drainage, but my attempts to contact them over the past ten years have been unsuccessful. Despite providing my contact information and evidence of mosquito infestations, I have not received any assistance. I am disappointed by the lack of response and the city's focus on new projects while neglecting crucial issues. As a disabled resident, it is challenging to address this matter with the city. I am determined to see this resolved for the safety of my home and community.
Thank you,
Steven Moore

On October 26, [redacted], while staying at the Stratosphere Hotel in Las Vegas, I experienced a severe reaction due to bites from bed bugs and developed scabies, originating from Room #[redacted]. Despite seeking medical attention and different diagnoses, my condition persisted for several months, leading to a challenging journey to recovery. The ongoing misdiagnoses, lack of proper reporting, and delayed testing of samples have prolonged my suffering. I have evidence in the form of videos, pictures, and medical records that highlight the negligence and errors in handling my case. My wife and I continue to endure the consequences of the mismanagement by medical professionals, leaving us in a frustrating and unresolved situation.

I have been using Methadone for pain management for Fibromyalgia for over 5 years, in addition to other medications. Recently, my doctor reduced my Methadone intake and added Oxycodone due to CDC guidelines on opioid crisis. However, the new dosage left me feeling overdosed and unable to function normally. After adjusting the timing and dosage on my own, I am still struggling with increased pain levels. During a recent doctor's appointment, my Methadone dosage was lowered again with the warning that it may decrease further. Despite expressing my concerns about increased pain, my doctor mentioned the CDC's stance on pain management and suggested less frequent dosing. Feeling overwhelmed, I sought advice on combining Oxycodone with Methadone but have been unable to reach my doctor or pharmacist for guidance. Managing Fibromyalgia for 25 years has become increasingly challenging with these changes. I am seeking clarification and assistance to ensure proper pain management.

I believe I have valuable insights into the potential causes of contaminated romaine lettuce. From my experience, I can identify various ways E. coli, hepatitis, and parasites may spread due to mishandling of produce from the farm to the store. I am knowledgeable about bloodborne pathogens and other disease-causing agents that could be transmitted through improper handling of romaine lettuce and other fresh produce. I am curious if the CDC is incorporating OSHA regulations and guidelines in their investigation into the issues with romaine lettuce.

I am reaching out on behalf of my aunt who was diagnosed with C.Diff after being treated at a hospital in Seattle, WA a year ago. Unfortunately, she was misdiagnosed initially and the treatment provided later was ineffective. Despite multiple attempts with fecal transplants, the C.Diff keeps coming back. Each time she returns to the hospital for treatment, the doctors' attitude towards her care is dismissive and lacking in compassion. This has been going on for over a year, and I am seeking advice on how to address this negligence. It is disheartening to see her health deteriorate without proper support from the medical staff. She was at Swedish Hospital on Broadway Ave. in November [redacted] when this all started. We are considering legal action and would appreciate any guidance on how to proceed. Thank you for your assistance.

I came to Cave Junction for work, where my tasks involved cleaning up dog waste. Unfortunately, there is a severe mouse problem, prompting the use of Decon to eliminate them. After being exposed to the poisoned mice, I started experiencing symptoms such as stomach cramps and vomiting. Upon noticing worms in my stool, I sought medical help at the local clinic which sent a sample to a lab for testing. As my condition deteriorated, I had to visit the ER, where I was prescribed medication costing $3,[redacted], which I cannot afford without insurance. The hospital provided me with insurance paperwork, which I have yet to fill out. I need guidance on how to proceed with getting the necessary medication and resolving this issue to ensure my recovery as my health is declining rapidly.

I am in Cave Junction for work and part of my duties involve cleaning up dog feces. Additionally, I have noticed worms in various areas around the property, including on my bike tires. The property also has rats. Since using Decon to eliminate them, I have been feeling ill with symptoms that seem related to the rodent situation. I went to the hospital after finding worms in my stool. The prescribed medication costs $3,[redacted], which I cannot afford without insurance. I am seeking advice on how to handle this situation and obtain the necessary medication to address the health issues caused by the worms. The situation is worsening, with worms now affecting my eyes, ears, nose, and lungs. I urgently need help to resolve this and stop the spread of the issue on the property.

My daughter has been diagnosed with Hepatitis A, and we are unsure of the source. She was employed at a small food establishment in the mall where she noted poor hygiene practices and unsanitary conditions. She has advised her boyfriend to seek medical evaluation as well. When she visited the health department for testing, they only informed her of the diagnosis without providing further guidance. I am worried about her health, especially as she is uninsured. I wonder why there hasn't been an investigation into the cause of her illness and what steps we should take next to address this situation.

I need guidance on where to send my medical records to demonstrate my current health issues. I am homebound due to continuous severe pain caused by spinal, coccyx, and sacral damage, as well as surgical trauma. During a recent stay at a budget room in Vergina, I used the indoor pool and woke up the next day with itchy, swollen spots on my head that caused hair loss. Upon returning to Ohio, my home was infested with fungus gnats that seemed to have nested in my head, leading me to move. I've experienced larvae jumping out of my head, which has been an ongoing issue with different bugs. Despite trying lice shampoo, the problem persists, with strange creatures resembling fish traits coming out. Medical professionals in my area have not taken my evidence seriously. I now have larvae in various parts of my body and need urgent assistance. Any advice on how to address this would be greatly appreciated.

I am a patient at Davita-Woburn Choate, MA. Today, 07/10/[redacted], when I arrived for my MWF appointment, I was shocked to discover a flood in the facility. Despite the flooded conditions, patients were still put on machines, forcing them to walk through 6-8 inches of water with a high risk of falling. Tragically, the FA and charge nurse were absent at 6AM. The head nurse had to call the FA, charge nurse, and building's maintenance department multiple times before their arrival at 9:30AM, despite being notified at 5AM. Only the building maintenance staff showed up, instructing the Davita team to keep mopping while I was left seated in the middle of the water pool. The situation became even more concerning when the building staff moved my chair while I was still attached to the machine, risking the leads in my chest being ripped out. I had to call out for help from the Davita staff to rectify this dangerous situation that needs immediate attention.
