It has been 45 days since my last California State Disability payment. My doctor submitted the necessary information to continue my disability claim. My status indicates that I am required to complete a Continued Disability Form to proceed with receiving further payments. Unfortunately, this form is not accessible in the online form library. It's frustrating that despite numerous attempts to contact them through the online claim status update, I have not received any response. The situation is similar when calling, as I keep getting a message about long wait times due to high call volumes with offices remaining closed. Last fall in [redacted], someone attempted to file an SDI claim using my SSN. Consequently, a flag was placed on my profile, making it impossible to apply online for SDI benefits in June [redacted]. I had to submit a paper form via mail. I have not received any updates on my claim, aside from a letter stating that I requested account updates on 8/7/[redacted]. Due to the fraud attempt, I have been locked out of my account since last October.
I urgently need to talk to an EDD representative who can call me, access my account, remove the fraud hold, update me on my legitimate SDI application status, and inform me about the timeline for my retro SDI payments. I have tried calling the SDI [redacted] number multiple times in the last two months but keep getting disconnected. It is incredibly frustrating. I have reached out to my local representative for assistance. EDD needs to resolve this issue promptly as I am owed money.

This is my second attempt using your website. I applied for SDI on 6/22/[redacted] and have not received any acknowledgment or payment yet. I have tried calling the SDI [redacted] number many times without success. I contacted my local representative last week but have not heard back. In July, I visited my local EDD office where I waited for almost three hours and was only given a paper form. They did not assist me further. The lack of support is frustrating. I expect to receive my payments promptly as a taxpayer who has contributed to SDI for years. The system needs more resources to handle claims efficiently and answer calls promptly.

After getting injured at work and taking leave, I was terminated upon returning for exceeding the allowed leave duration. From May 9 to August 26, I requested paid family leave, providing documents from my employer and myself. I have yet to receive any response from EDD regarding my application. I recently received Form DE 6363D Rev. 2 (10-21) in the mail, but I'm uncertain how to fill it out. The form seems more geared towards employees of traditional businesses, while I am the sole owner and employee of my one-person S-Corp, Bumgardner Biological Consulting. My company doesn't have a standard payroll system, as I receive a monthly salary along with additional payments when receivables surpass the monthly amount. I submitted all necessary documents and completed the form from my doctor. It has been three months, and there has been no response. The automated system does not show my claim as pending, even though I have the receipt number confirming its submission.
I would appreciate SDI contacting me to resolve this matter promptly by either processing my claim or issuing the payment. Despite calling multiple times at 8:01, I have been unable to speak to a representative because the lines are already at full capacity, leading to my calls being disconnected.

I have been attempting to reach California Disability Services for the past two weeks. Their phone lines are continuously occupied, making it impossible to connect with a representative. Often, the calls end abruptly without further assistance or, as it happened today, they promise a callback that never materializes. It would be beneficial if they could refrain from disconnecting the calls and instead let customers wait in the queue; many of us would be willing to remain on hold until assisted. Alternatively, following through on the promised return calls would be greatly appreciated. A brief explanation of my issue, which is not easily addressed on their website, would promptly resolve the matter.

I completed my SDI Online application on May 11, [redacted]. Due to my doctor being located outside the US, the physician certification form had to be filled out and sent by mail. I completed this on May 23, [redacted]. I have tried reaching out to an SDI representative multiple times during business hours but have been unsuccessful. I would appreciate receiving any further communication via email to reduce waiting time compared to traditional mail.
