I have made multiple calls to [redacted] and have encountered excessive prompts that do not address the issue I have been attempting to resolve since April [redacted]. The customer service experience has been very poor overall. After reaching a live person, I was informed that I had contacted the wrong department and was instructed to call the same number again. The prompts are unclear, making it difficult to select the appropriate one. I am seeking assistance with an open claim for my late husband. The Bank of America Estate Department Case Manager suggested contacting EDD. I have been struggling to resolve this matter since April [redacted] when I first received a letter from the Bank of America Estate Department. I have shared my frustrating experience with EDD to BOA. I am looking for guidance on which prompt to select to address this issue effectively.

I need to speak with an SDI agent regarding my non-automatic pay status. I have completed the online form as required every two weeks, but the website still prompts me to submit the form to receive payment. I am seeking assistance to ensure I receive my payment promptly. Additionally, I am interested in transitioning to automatic pay but cannot find any information on the SDI website distinguishing the qualifications for automatic pay from non-automatic pay status. There is a lack of details regarding pay status on the website. After emailing the SDI for information, I received an automated response citing their busy schedule and limited responses unless further information is needed. I also received a notice regarding a change in my aid start date, to which I submitted an online appeal. I would like to verify that the correct aid start date is being used for my case.

I submitted my temporary disability forms online on 9-10-[redacted]. My account number is [redacted].
My primary care physician, Dr. M. Huie from Orinda, CA, sent in the forms in October [redacted], but the online system still shows my claim as pending the medical provider form.
I also completed a paper form, which was sent along with my physician's submission.
I have been attempting to reach a representative by phone for months, but I have only encountered automated systems. Could someone please contact me to provide guidance on what is required to progress with my claim?
Thank you for your assistance,
T. Cheney

I am experiencing issues with transitioning from workers' compensation to California disability. Despite receiving instructions from my case representative Esther, I am unable to reach her at [redacted]. Every time I call, I am faced with a full queue message and unable to leave a voicemail. Even when attempting to speak with a representative by dialing "0," I am told that option is not available. After numerous unsuccessful attempts, I contacted Disability Insurance and after a long wait, was informed by a representative named Ralph that they couldn't assist due to the specialized nature of workers' compensation. I am feeling overwhelmed as the deadline provided in the email is fast approaching on 1/20/20. Despite my doctor's office receiving approval, navigating California Disability Insurance has been challenging. I am seeking guidance on how to proceed in this frustrating situation.

I have been trying for over a month to resolve the issue with my November back payment and our January payment. The payment was doubled up incorrectly. Please, someone competent, contact me to rectify this mistake. It is frustrating to deal with this error. I insist on speaking with a supervisor rather than waiting in the phone queue. Kindly have a supervisor reach out to me to address this matter. My contact number is [redacted].

1) I need help attaching something to my DE1000A form that I didn't have at the time of submission. Can anyone advise on how to do this after the initial submission?
2) When trying to submit form 2500A Cert for Continuation of benefits, I received an error stating that the recovery date must be today or before today. However, I have not yet recovered. How can I request an extension to submit the form successfully?
3) I am unsure about the effective date for a claim. Is it the first day you receive disability benefits or one week before receiving them? Can someone clarify this for me?

I am Scott C. and I recently faced a serious illness that almost took my life. Despite my health improvements, I wish to handle my disability matters independently. Previously, my sister Kellie C. assisted me, but our relationship has deteriorated, and I now want my mother, BettyAnn C., to be the secondary contact. I am concerned about my sister's threats to interfere with my benefits. I need guidance on removing her authority to act on my behalf and ensure only I have control over my personal affairs. Thank you for your assistance.

Due to the Covid outbreak, a family member, undergoing chemotherapy for an immune condition, faced challenges at work due to safety concerns. Although set to work from home, the company couldn't facilitate it, leading to state disability approval despite no physical ailment. The concern is the 60% payment which might last a year until a vaccine is available. The query: Can the prohibition on seeking part-time remote work while on disability be waived? This would help cover expenses as the reduced payment might not be sufficient. Seeking exceptions during these unprecedented times to bridge the financial gap. Grateful for any guidance.

My company van was broken into and my wallet was stolen, containing my California Driver's License, Social Security card, and bank card. The thief used my bank card at various stores and even filed for unemployment in my name, collecting over $[redacted]. I have reported this to the local authorities, the Fair Trade Commission, Bank of America, IRS, but I am having trouble reaching EDD. The issue is that my disability claim number is the same as the fraudulent EDD claim. I need SDI to correct this mistake promptly and return the funds to my account. I am scheduled for surgery tomorrow and have been trying to get help since July 20th. I am going to be in recovery for some time and unable to pay my bills due to this situation.

I have been receiving California State Disability Insurance for about 5 months. Initially, my doctor suggested I wait until 03/01/[redacted] before returning to work.
Now, she has approved my return on a part-time schedule (3 days a week instead of my usual 5 days) before going back full-time on 03/01/[redacted]. I am unable to find a way to update my claim online to inform the Employment Development Department about my new part-time work status.
Additionally, I have not been able to locate an option to speak with a representative through the standard [redacted] helpline.
Thank you, Mike

I submitted a SDI claim to EDD on 11/09/[redacted] via Certified Mail, but I have not received any updates. Despite sending the forms and receiving the certified mail return card, I have not heard back from EDD in 11 weeks. I have attempted multiple times to contact EDD but have been unable to speak with a representative. I am uncertain about the status of my claim and would like guidance on how to proceed and resolve this issue. Your help with this matter is greatly appreciated. Thank you.

My husband's doctor completed the medical form and submitted it. Later, he was informed of an issue with his SSN that he promptly resolved with the social security office. Despite completing all necessary steps, we have not heard back from the Disability office. No deposit has been made to his Bank of America account. This process has been ongoing since August, causing financial strain and impacting our ability to pay bills and rent. I understand the challenges the office may be facing, but it is frustrating not to have the opportunity to speak to someone for clarification and resolution.

I had an accident on 2/05/[redacted] and opened an EDD account on 2/6/[redacted]. I tried to get verified through ID.me and submitted my documents, but all I received were emails saying they were overwhelmed and would get back to me shortly. I haven't been able to work during this time and I'm living off my totaled motorcycle equity, tax refund, and savings. My account now only has $13.40 left. After my doctor suggested SDI insurance, I applied online, but my doctor couldn't complete his portion online. I had to pick up a paper application at his office, completed my portion again, and mailed it to the Santa Ana office. Despite numerous attempts, I haven't received any response via email or phone calls to EDD. It's frustrating not being able to access the insurance I need to protect my family and pay my bills. Many others seem to be facing similar challenges when I look online.

I work as a patrol officer for the Golden Gate Bridge Highway and Transportation District. I was injured on the job on 07/11/[redacted] and have been unable to work since. I am currently in the Permanent Stationary stage, but my doctor has not completed the necessary report yet. My next doctor's appointment is scheduled for the 28th of October, and I am expected to reach Permanent Stationary status at that time. I initially applied for temporary disability on the 4th of October [redacted], but my hip doctor extended my treatment, keeping me off work until the 28th of October. My original application for temporary disability was denied because I was still receiving payments from my worker's compensation insurance. However, these payments will cease by the 28th of October [redacted].

I need help with my EDD certification. I was released back to work on 10/21 by my inpatient provider, Dr. Westervelt. I returned to work for four full-time days and then went on leave again from 10/27 to 11/29. Shouldn't I only need to file a claim from 10/27 since I'm not inpatient anymore? I went on leave again on 12/1 under the same doctor. I can't reach EDD by phone and am very worried. I haven't had income since early September and applied for COVID relief. I'm facing eviction and feeling scared.

My disability claim was denied as I have a workers’ comp case, and you did not receive any documentation confirming I was not receiving it. Despite not currently receiving workers’ comp benefits, my former employer failed to provide a delay or denial letter to me or my attorney. Despite repeated requests from various individuals at the law office and the insurance administrator, we have yet to obtain the necessary letter. The situation is frustrating, especially as the insurance administrator transitioned the case to someone new without addressing this outstanding issue, causing further delays. I have a deposition notice pending. Do you think this would be sufficient to demonstrate my lack of workers’ comp benefits, or do you recommend obtaining the letter?

I recently received three 'Notice to Employer of a Disability Insurance Claim Filed' forms in just two days. One form states that my husband filed the claim, another lists me as the claim filer, and the third one names someone who has never been employed by us. My husband and I have never initiated any claim, so I am worried that there may be fraudulent activity at play.
Despite my numerous attempts, I have been unable to reach a person over the phone. Please assist me in resolving this matter promptly.
Thank you,
Bobbie E.

I was informed that I was disqualified because I used the same date. I have submitted an appeal form but have not received a response yet. My answers in the form were genuine and accurate. On October 28, [redacted], I was in an accident on my way home from work that nearly took my life. I was rushed to the hospital that same day, and I am still recovering. I applied because I am currently in the hospital and in need of assistance.

I submitted my certification on 11/2/[redacted] but have not received any payment yet. I have not received any type of payment since 11/10/[redacted], and my return-to-work date is 01/01/[redacted]. I have my receipt number, but the only mistake I made was putting 01/01/[redacted]. I have already corrected it. I have tried calling, but it's only an automated system. I have been requesting updates since around 11/28/[redacted], with no response. The claim history states I need to submit certification to continue benefits, which I have done, but it does not provide a new one to fill out. I feel stuck in limbo, and this situation has been causing me hardship. With only $10 to my name, I urgently need this resolved as it's causing me significant anxiety and depression.

I submitted my benefit request online yesterday afternoon but encountered an issue with my primary physician not being able to fill out the form electronically. She will be able to complete the paper form and mail it back tomorrow. I am concerned about the timing as my medical leave started on Nov 2nd. My psychologist, who prescribed an additional month of leave following the initial two weeks, is willing to provide an online doctor's certification. Can I send the receipt number to my psychologist for completion? Also, is it acceptable for my primary physician to mail the paper form? Thank you.
