Account holder name, Billing address, Account phone number, and Email address on the account

Betty LeClare acct. number ************ has been a subscriber for a number of years and I sent a request to continue her membership around June **, I sent application received in mail along with credit card number. As of today (*******) Neither I or Betty has received any info as to the continuation of this subscription nor has she received any issue since the **th.
