Account holder name, Policy number, Date of birth, Billing address, and Email address on the account

HELP!!!**I’m trying to get my Explanation of Benefits mailed to me in proper form. This is my sixth attempt to do so. First one was on me, as I completed the proper form to add my brother as my personal representative as a contingency. Unfortunately, CIGNA’s mindset is that any named personal rep overrides the insured’s current parti****tion. The last four attempts have been with the consultation and advice of various CIGNA phone representatives. Last CIGNA employee I talked to - twice - was Erica Black @ ***-***-****, who’s actually in CIGNA’s South Carolina office despite her *** area code. Now I’ve left two messages for her to return my call and I haven’t gotten a response. Originally my EOB were mailed to my brother in Indianapolis and despite explaining the problem several times to several CIGNA reps the EOB are now coming to my Flordia address but addressed to my brother.
