He selected UPS air shipping to my address to ensure timely delivery. Since he does not have his own email, he used mine for communication. His name is M.E. Montalbano, born in Salinas, CA.
I am D.K. Austreng, born in Exeter, CA (Visalia County). I have moved and not updated my address with the DMV. Both of our official addresses are [redacted] Cedar Street, Mount Shasta, CA [redacted], but we asked for the certificates to be sent to my Sacramento address. Will this be an issue?

I paid $49.00 to Vital Records Badalona and have not yet received my birth certificate. I received an email with a birth certificate application and two pages of instructions. The application says to mail the form to a P.O. Box in El Paso, Texas, while the instructions from VitalChek state to send it certified/return receipt to Colorado, my birth state, along with a $20.00 check. I received an order confirmation number, but I am unsure if it's from VitalChek or Colorado Vital Records. I am confused about where to send the application and how much to include, payable by check.

In the last two days, I purchased a death certificate for my father and received a document stating there was no record of his death, which I knew was incorrect. I searched again and eventually found his death certificate from Florida. The search I requested from VitalChek for the death year in Florida ([redacted]) yielded no results, although I eventually located the necessary information about the day and place of his death. I am requesting a full refund as the service I paid for was not provided, and I only found my father's death certificate by chance.

I purchased a birth certificate for my daughter initially, costing almost $60 and was informed it would take 60-75 days. Shortly after, I realized I could expedite the process for nearly $80. I have been attempting to cancel the first order for over a week unsuccessfully. Now, I've been notified that I can't receive the expedited document until I pay an extra $50 fee, despite sending a $25 rush fee along with court documentation instead of the correct document. I wish to transfer the payment from the first order I'm trying to cancel to the second order, which is already well into processing. I possess both order numbers and pins for reference if needed. I urgently require my daughter's birth certificate by August 24th for various matters. The court provided a certificate of live birth when I obtained custody last year, which I have now discovered is inadequate. I have already paid all required fees; however, due to unique circumstances and a misunderstanding, I seek your assistance in obtaining this document promptly.

On August 2, [redacted], a charge of $54.00 appeared on my Chase credit card ending in [redacted]. This charge was a mistake. While trying to obtain my birth certificate, I visited a website and thought the initial payment covered everything. However, after filling out the information and printing the form, I realized additional charges totaling $30 were required with the application. I tried to cancel my service as I did not wish to proceed. $89 is excessive for a $10 birth certificate request. I did not submit the form and did not utilize the service. Please refund the $54 charge.

Since June, I've been trying to obtain my birth certificate from Los Angeles County. Initially, I thought my corrected order #[redacted]98 was complete, along with the submitted documents. My attorney faxed the necessary paperwork, but I received a new order number (#[redacted]93) indicating the order was incomplete. Despite sending follow-up emails, I was informed I had filed too many orders previously.
I listed my name differently across documents, causing confusion. My urgent need for the birth certificate is to renew my license and passport. I may need to legally adjust my first name.

I am seeking information on how to obtain a birth certificate for an individual who was adopted. The birth occurred in Polk County, Des Moines, Iowa, and the adoption was completed and filed in the Superior Court of California, County of San Joaquin. The child was born on April 26, [redacted], and the adoption paperwork was finalized on February 4, [redacted], in the Superior Court of San Joaquin County, California. The individual, now 21 years old, needs their birth certificate. Unfortunately, the adopted mother, who recently passed away, had the birth certificate but it is currently missing. I recently ordered a set of death certificates that were unfortunately lost by UPS. After filing a claim with UPS and tracking the package with number 1Z038Y[redacted]55, VitalChek informed me that they are prepared to provide a full refund for the order due to UPS acknowledging the loss. I reached out to VitalChek on January 5th about my refund and confirmed my payment information was still valid. Despite this confirmation, I have yet to receive the refund of $87.45. I am looking forward to resolving this issue promptly.

My mother-in-law and I needed to obtain a certified birth certificate from Iowa for my late husband. Iowa Vital Records contacted me and informed me that it should be filed as a copy of another person's birth certificate. Despite their help, I experienced difficulty getting transferred to the necessary customer service lines within your company. I made several attempts to contact them to make the required changes to the report.
Iowa's Vital Records department confirmed that we could obtain the certified copy. We tried to upload my mother-in-law's Iowa driver's license this weekend, but encountered some technical issues. We require the certificate for legal, insurance, and pension matters. I have noticed that the payment has been deducted from my account and want to clarify the requester of the records.

Good morning. I am unsure if you can assist me, as about 20 years ago, immigration confiscated my documents as they were deemed insufficient. During that period, a law was passed stating that any undocumented person living in Brownsville was a U.S. citizen. Feeling very isolated, I couldn't take action. Now, I am striving immensely to help my grown daughter. I am attempting to secure her dual citizenship because she does not speak any English, has never been to the U.S., and lacks any U.S. documents. I submitted an order in Yakima County, but have not received a response via email. Hence, I am seeking alternate methods by providing my SSN for reference. Although she does have one, it seems as if she has never owned it due to immigration seizing my documents in Brownsville, TX. I implore for your assistance and support in this urgent matter. I am seeking assistance in tracking my birth certificate. I have already made payments through my Direct Express account to Vital Records in Stockton, CA. The Vital Records office charged me $32 for a certified copy, as well as two processing fees of $12 each through VitalCheck.com. However, the County Recorder in Stockton, CA has informed me that they have not yet received any documents. It has been a week, and my attempts to inquire about my order over the phone have been unsuccessful. I faced long hold times, only to be asked to call back due to high call volumes, despite being told they are open 24/7. Unfortunately, I have not been able to reach a representative. Thank you. R.
