I am writing to address the issues I have encountered with my premium payments and coverage under the group contract with The IBM Corporation. I recently received conflicting information regarding the status of my insurance, leading to confusion and concern.
On August 19, [redacted], I was informed that my coverage had lapsed despite making a payment on June 30, [redacted]. Subsequent calls to customer service resulted in further confusion, with conflicting information on the status of my policy. Despite multiple attempts to resolve these discrepancies, my concerns remain unaddressed.

After our mother passed away in May [redacted], we discovered a life insurance policy. While two brothers promptly received their benefit checks, I faced delays and received mine after 9 weeks. Another brother, however, is still waiting for his benefit check four months later. Despite numerous calls on his behalf, the issue remains unresolved. A letter from a general manager promised his check within 5-7 days, but it has not arrived. I am considering involving our mother's attorney to address this matter. I have documented all conversations with the company and have names and dates of contact if necessary. My mother was Genevieve E. Fleury, and my brother is Donald J. Phillips. On Friday, I received a call from a man claiming to be a Prudential representative for eastern Tennessee. He mentioned that my late husband was listed as the primary on my life insurance policy, with my deceased father as the secondary beneficiary. I had previously updated this information when my husband passed away, believing I had made the necessary changes. The caller provided the number [redacted]. I am verifying the authenticity of this call to ensure it is not a scam before reaching out to make any alterations.

I established a life insurance trust over 20 years ago for Gail McLeod Stark, naming my children Julia McLeod Stark and Dustin Eliot Stark as beneficiaries. Orphalee Smith, my former accountant, managed the trust, making quarterly $15,[redacted] payments for the $60,[redacted] annual fees. As Orphalee is aging, I am uncertain if she knows how to access the policy. I recently submitted an online life insurance claim with Claim ID: AMRL33HV. However, when attempting to print the online claim form, I only see blank spaces. I'm unsure how to obtain a printable form.
Upon calling [redacted], I encountered a delay before a woman answered. Despite explaining my issue, she proceeded with a phone claim instead of resolving the printing problem. I still require a printed copy of the claim for my records. It appears this was feasible before but now it's not.

I received a notification that my 1099s were available online. However, when I tried to access them, I encountered a "[redacted] Forbidden" error. I live in a small rural town, and I'm unsure if the issue is with our Internet connection. I can receive emails without any problem, but I am unable to log into my account to view the 1099s. I believe I faced a similar problem last year, and a helpful Prudential representative sent the 1099s via email.
I would appreciate your assistance with this matter. I opted for paperless communication and find it convenient to use services such as this chat line.

I recently relocated to Iowa and updated my address with Prudential. Despite confirming my new address with them, I have not received my $31.00 check for October, and now it's already November. I provided them with my updated address for general delivery in Story City, Iowa [redacted]. The contract number related to this issue is CSC[redacted]44, and the owner is Eva Acton, my grandmother who left it for me. Upon my last communication with Prudential, they assured me that the October check would reach my new address within 7 days, but unfortunately, it has not arrived yet.
