I had a burial policy for $5,[redacted] on my daughter, Patricia A. H., with Policy Number [redacted]-[redacted]-[redacted]-6. Her date of birth is 03/07/[redacted], and she passed away on September 4, [redacted]. It has taken two months to have this policy processed and I am yet to receive the check. The funeral home supposedly submitted two certified death certificates, but there were issues with not receiving them. Eventually, I had to provide additional documentation including my husband's death certificate and proof of identity as Norma Jean T. I am frustrated with the delay and lack of communication. Waiting two months for a payout on a policy I've had for fourteen years is unacceptable. This experience is making me reconsider continuing with your company for my other daughter's burial policy. The service I have received does not meet my expectations based on previous positive experiences with a policy my husband had. I am disappointed and would not recommend your services to others.

I am writing to request the removal of my husband, E. Carlos, whose date of birth is 7/9/45, from this policy. Unbeknownst to me, he already has dental insurance.
The initial payment covering both of us is set for automatic deduction from my checking account on Friday, July 12. Please update the policy to include ONLY MYSELF.
