Two weeks ago, I submitted the surrender, loan, and dividend form for policy No.[redacted] under my name, Rex C. Hughey, residing at [redacted] Focis St. Metairie, LA [redacted], with a birthdate of November 3rd, [redacted]. Today, when inquiring about the status of my request, I was informed that my social security number was missing on the form. Upon review, it was confirmed that the SS number was indeed included in the correct section. I am concerned that my form may have been delayed unnecessarily. I have been informed that it will be reviewed again. My main query is regarding the current status of my request and when can I anticipate receiving the cash value check for a policy I have held for over 60 years, faithfully paying the premiums. Please provide me with an update at your earliest convenience.

I have four policies with you and I would like to know the balances of each one for IRA, non-qualified, and a life annuity. I believe one policy was delivered to the wrong address as it is listed incorrectly on file. The correct address should be [redacted] N. Branch St, Macomb, MI [redacted]. The policy numbers are [redacted] (deferred annuity) and others are sent to the correct address. I also need a statement of my IRA withdrawals for RMDS over the last 10 years. I usually don't have trouble getting information for my tax person. Are all withdrawals made on the IRAs treated as RMDS? I am hearing impaired and prefer email communication. I called again, speaking with Allan, and reiterated my desire to cancel, which he assured me he would process. I hope this issue of cancellation can be resolved promptly to avoid further stress.
