Hello, I am Jamie Hampton, a long-time member of Navy Federal Credit Union. While reviewing my account, I noticed two CMFG Life Insurance policies. One for $15.00 is familiar, but the other one for $79.00 is new to me since the passing of my wife, Robin Hampton, in April [redacted]. I am currently working overseas in Tel Aviv, Israel. I am Catherine S. from Hudson, NC. My mother Doris M. L. shows payments to your company on her Prosperity Bank statements in TX. I am trying to settle her estate and need help as the executrix of her will. I don't have the policy numbers but she pays $54.86 and $26.03 monthly. I have held this insurance for many years and I need a copy of the policy and information on my listed beneficiary. I am currently 71 years old and I am unsure if the life insurance policy is still active in case of my passing. I would like to have the policy numbers and mailing address on file for my beneficiary. I have been waiting for months since before November [redacted] for these documents, but they have not arrived. Kindly send me the necessary copies and addresses promptly. I am unsure if my previous message was successfully submitted since I encountered an issue with the "continue" button. Therefore, I am rewriting my request. My late mother, Elisa M. Carter, passed away in April. She held an NEFCU banking account from which CMFG Life Insurance Co. deducted premiums. I have not located any documentation from your company, so I am unaware of the product being paid for. I would like to ascertain the type of policy she held, its issuance date, current status, coverage amount, listed beneficiaries, and whether any claims need to be initiated. In case the policy is inactive, I seek clarification as to why premiums are still being withdrawn from her account and for how long this has been occurring. I kindly request this information and any necessary forms to be sent to her registered home address.

I manage our finances, and I made an error assuming my husband's $15 payment was to CMFG for insurance. Recently, he mentioned we don't have CMFG insurance. This recurring charge is fraudulent, ongoing for about a year now. I need to stop it from debiting my bank account linked to my husband, Colten Lindquist's, America First checking account. We have an Accident Death and Dismemberment (AD&D) insurance plan that was acquired through AF Credit Union several years ago. Since we have moved, we no longer receive updated information on the policy. Despite the quarterly payments being deducted from our AF Credit Union checking account, we do not have the account number accessible. It appears that there may have been a company name change. We are looking to obtain our account number, a summary of the coverage provided by the policy, and instructions on how to stop the automatic withdrawals if the coverage is no longer suitable for us. A few years ago, my father suffered a stroke, and we were unaware of this policy. Funds were withdrawn from his savings, and we had to reorganize his finances for his nursing home care as he is unable to care for himself. It seems the withdrawals have ceased. Can we reinstate his policy? Due to his inability to communicate, this situation occurred in error. Please ensure prompt communication regarding this issue.
