I recently switched from a PPO to Blue Max HMO on 8/13/[redacted]. My ID number is XOH[redacted]29, and my Medical Group Number is [redacted]. My son, who is attending college in Minnesota, needs an MRI for an ongoing ankle issue. He has an appointment with Summit Orthopedics in the Minneapolis/St. Paul area scheduled for tomorrow, and they confirmed that no Authorization is needed based on the Reference Call made with the provided ID number. My concern is whether the MRI will be covered since Summit is a specialist, and there is no referral on file as he began treatment under the previous PPO ID XOF[redacted]29 Group PA1333. I hope to avoid any unexpected costs associated with the MRI.

I recently received a bill from Kurtz Ambulance Service for my deceased brother, Richard S., for a 3.8-mile transport to St. Joe's Hospital in Joliet on 10/20/17 amounting to $1,[redacted].00. The bill arrived one year later, dated 10/8/18, citing a claim denial by BCBS on 1/26/18 due to coding errors. Despite resubmission, it was denied again. Richard needed urgent transport due to a stroke prior to his passing on 11/25/17. I urge for a review and reconsideration of this excessive bill. I am currently attempting to reach BCBS for clarification as his POA/Executor. Kindly keep me informed of any progress. Jonine S. [redacted].

My daughter has been incorrectly enrolled in a BCBS Medicare plan through your company, causing denials for her medical services. She only had United Health Care until November [redacted]. I was informed that your company is the only one that can fix this issue. Her name is Terri Goodson, born on February 25, [redacted], with Medicare number 3CH0-RV5-RF38 under Part A and B, and Medicaid RIN number [redacted]76. She has only had Medicare and Medicaid since December [redacted]. Denials have occurred at OSF Hospital and her primary doctor in Rockford, IL. Please correct this to avoid further issues. Thank you. Cheryl Olson, Mother, Guardian & SS Payee, [redacted] 14th Ave, Belvidere, IL [redacted], Contact: [redacted].Terri has disabilities, so I act on her behalf.

I've been a member for over 20 years through my husband, Michael A. Judith. We are now divorced and have separated our accounts. I received your bill some time ago, but I have not yet received a card. Payment is due in one week on the 1st, and I want to create an online account for automatic payments. I require the card number to proceed. I also need my account number for setting up the online account and require the card promptly. If helpful, the old card with my ex-husband's account number is XJWH[redacted]5. I am eager to speak with a representative soon. Thank you.

I'm currently covered under my father's BCBS plan, and I also have secondary insurance through the Illinois Medical Card due to being a single mother. Each time I've visited a doctor, BCBS hasn't covered any expenses because of the dual insurance situation. They've requested information about my secondary insurance, causing confusion and resulting in bills that I'm now responsible for. My name is Kelsey Stoehr, born on 10/20/[redacted], and my father is Joseph Stoehr, born on 9/05/[redacted]. My BCBS details are as follows: Card # CIF[redacted]82, Group # P79444, BCBS of Illinois PPO through the Construction Industry Welfare Fund of Rockford. My Medical Card number is [redacted]41, solely serving as secondary coverage for expenses not covered by BCBS.

Good morning,
We are currently seeking a medication, Hydroxyprogesterone, for a patient with BCBS of Illinois. This process has been ongoing for over two weeks. Each referral has led us to another specialty pharmacy, from CVS Caremark to Prime Therapeutic, Alliance Rx, and Accredo. Unfortunately, all have stated they are unable to fill the prescription. We urgently need assistance in locating a pharmacy that can fulfill this prescription for the patient, Cassandra D., with a history of preterm labor, entering her 16th week of pregnancy next week. Patient advocacy to identify the appropriate pharmacy would be greatly appreciated. The prescription is typically covered under the medical benefit, and we have approval ready to provide. Thank you for your prompt help.

I had my husband on my Bronze PPO [redacted] policy until 1/1/23 when he switched to Medicare. I contacted BCBS on December 1 to remove him from my policy starting 1/1/23 while keeping the same insurance. Despite their confirmation on December 3, they deducted $2,[redacted].13 from my account on 1/2/[redacted] instead of the correct $1,[redacted].83 just for my coverage. I made multiple calls in December to ensure the billing adjustment for January, with assurances it would be corrected by 1/1/23. However, I'm still facing the overcharge of $1,[redacted].30 and can't seem to find anyone at BCBS to assist in resolving this issue and refunding the excess amount.
