I have held an extended care policy with American Fidelity for approximately 17 to 18 years. Now at 75 years old, I need to file a claim due to health issues after battling pneumonia and Rocky Mountain Spotted Fever. I have had support from friends for nearly 3 months. When contacting the phone number on my policy, we were first directed to another company and then back to American Fidelity, where they could not locate my policy details. The policy number is [redacted]-[redacted], issued on 6/1/[redacted] to J.F. Sizemore. Payments have been deducted from my Farmers State Bank account in Booneville, Kentucky to AFA Group/LTCG, Inc. I am struggling to find assistance over the phone to make a claim for the in-home care covered under my policy. I urgently need to be connected with the right people to process this claim as I am unable to manage my daily needs without help due to my health condition.
