What documents do I need to bring for my first visit?

For your first visit to our Veterans Medical Clinic, please remember to bring several important documents. First and foremost, bring your valid identification, such as a driver's license or military ID card. Additionally, please bring along your Veterans Health Identification Card or any other proof of your military service. It is vital to bring your current insurance card, whether it's through the VA, Medicare, or any other provider, as well as any relevant medical records or documentation pertaining to previous treatments. Providing a list of medications you are currently taking, including any over-the-counter drugs or supplements, will also be beneficial. Lastly, if you have any specific medical concerns or questions, a list of these beforehand will assist in optimizing your visit. Remembering these documents and information will help us provide you with the best possible care.
