I sent my form W-8BEN to State Street to meet the May 31, [redacted] deadline. It was returned to me on June 24, [redacted], causing State Street to halt my pension payments for four months. After submitting a new form on September 16, [redacted], I received a letter from them on November 24th, approximately 70 days later, requesting an affidavit confirming my residence in Brazil. State Street informed me they couldn't reimburse $2,[redacted].40, as payment was only possible in the [redacted] fiscal year. I received the November 24th letter after traveling abroad and signed and returned the affidavit on January 14, [redacted]. Amanda Hall from AonHewitt informed State Street of my absence before my departure on December 14th. The USPS failed to deliver my form on time, leading to this penalty, and the 70-day delay in State Street contacting me does not reflect well on their service.
