My mother passed away on May 26th, [redacted], and she had an annuity with Prudential naming myself and my two sisters as beneficiaries. I have contacted Prudential multiple times to inform them of her passing and have submitted all the necessary documents, including the death certificate. Despite this, they claim not to have received the paperwork and have not yet processed the payout. I have sent the documents via fax, email, and FedEx several times and have spoken to numerous representatives, keeping detailed records of each interaction. I am frustrated with the delay in receiving the payout and would appreciate any assistance in resolving this matter.

I recently received a letter from Prudential Annuities indicating that some of the checks they sent to me were returned by the Post Office as undeliverable. The address they have on file for me as Robert D Blevins is incorrect.
The correct address should be:
Robert D Blevins
[redacted] S. Waverly St
Kennewick, Washington [redacted]
I used to work for Willard Johnson over 35 years ago and believe he may have had some insurance policies for the employees. Unfortunately, I do not have his account number or any additional details.
Thank you,
RDB
