I need some advice on a dilemma I am facing. I have been using Humalog since [redacted] and have been on a Medtronic pump for the last 6 years. Unfortunately, I am currently out of insulin as my new insurance provider is not covering my Humalog and wants me to switch to NovoLog, which I have previously tried and do not tolerate well. I have been without insulin for two weeks now, and despite my efforts, the insurance is not willing to make an exception. Who should I reach out to for assistance with this situation?

The Medtronic representative was scheduled to assist Dr. Kasendorf with setting up the intrathecal pain pump for the patient in room [redacted] at Scripps Memorial Hospital in La Jolla, California. Unfortunately, the Medtronic rep did not appear, causing the doctor to be unable to administer a medication bolus. It is unclear if the doctor was familiar with this specific pain pump. I am seeking information on why the Medtronic representative did not attend as planned.

I have experienced recurring issues with the Guardian sensors. The last three sensors I attempted to calibrate failed to accept the initial calibration input, leading to me replacing them. This is the first time I received three faulty sensors in one shipment. I would like an explanation for why this occurs when I input my blood sugar levels. I kindly request a replacement for the three malfunctioning sensors. The fourth sensor is functioning properly. My pump is the 640G with the serial number NG[redacted]H. For further correspondence, please reach me at [redacted] or [redacted]. Address: April Wortham [redacted] Bay Front Road Sparrows Point, MD [redacted]

I recently had a conversation with a senior administrator at Medtronic named Ms. Pollack. She assured me that she is well-known within the company and has been very helpful. I'm trying to reach out to her again but unfortunately do not have her contact information. After making several phone calls in an attempt to find Ms. Pollack, I am seeking assistance from anyone in customer service who can help me address my concern if she is not available. Thank you. -Jeffrey Silverstein

I am experiencing ongoing issues with my Medtronic [redacted] Insulin Pump and am now required to complete an ABN form to receive necessary supplies. The wording in Sections D and E of the form is inaccurate, and attempts to correct it have been unsuccessful. Unfortunately, the document team is unreachable by phone, which I find frustrating. I am seeking assistance from a supervisor in the Document Department or Legal Department to rectify this matter promptly. It is vital that the ABN reflects correct information to proceed with obtaining the supplies. Your urgent attention to this issue would be greatly appreciated. Thank you for your assistance. - C. M.
