Dear Concerned Party, I am seeking guidance regarding transferring my husband's healthcare from Stanford Hospital and Clinics as we relocate to Arizona. I am interested in learning about transitioning to the Mayo Healthcare system. Previously, at Stanford, we benefited from the "Coordinated Health Care" service for employees and spouses managing multiple diagnoses and medications. Though I realize this service may not transfer, I am keen to explore how we can smoothly transition his care. After 31 years with Stanford, I am reluctant to start anew but need to coordinate care for my husband who deals with various medical issues including cardio, pulmonary, endocrine, bladder cancer, and primary care concerns.
