I have a Medicare Supplement Plan G and have had it since I turned 70. I need an insurance card sent to me for my upcoming cataract surgery on January 4, [redacted]. I've never filed a claim or used my Medicare Part B or the Supplement Plan G. My name is Nancy S. Miller, and I live at [redacted] Maddox Road, Augusta, GA, [redacted]. My birthdate is June 11, [redacted], and the last four digits of my social are [redacted]. I've been unable to log in online since I purchased the plan, but payments are automatic. We, an out-of-network provider, have been attempting to upload our provider tax IDs and NPI to Anthem Blue Cross and Blue Shield in Georgia since May 8, [redacted], with no success to date. We have encountered obstacles such as being informed that they do not communicate with out-of-network providers and that our providers are not in their database. Despite completing all necessary requirements and receiving correspondence stating our case was resolved, our claims are still being rejected, necessitating W9 forms. Despite our efforts through payer chat and phone calls, we have received no assistance. Our next step is to file complaints with the states of NY, LA, and GA as Anthem's actions have caused [redacted] claims to be denied for untimely submission. Any additional guidance would be greatly appreciated.

I have been waiting for almost two years for reimbursement on medical claims totaling almost $[redacted]. Both my fiancé and I spoke with an agent who confirmed that my claims were approved, but I have not received any payment yet. Currently, I am still paying $[redacted] a month for the doctor's visits and almost $[redacted] for medication that should have been covered, with the agreement to pay only $45 a month.
