I had a visit with Minute Clinic at the Dow Road, Wilmington NC on ********. My insurance is through Medicare Health Insurance (primary - Medicare) and Emblem Health (secondary). I received a notice from Empire Health that they need a complete itemized claim form from the primary carrier, which would include patient's name, date of birth, diagnosis code, procedure code, date of service, provider's Tax ID Number, name and address, and the place of service for all charges from the primary carrier. I do not have this information, which causes me to think that Minute Clinic did not submit this information to the primary carrier. I would appreciate you checking on this for me.
