On September 23, [redacted], my wife paid a $[redacted].40 bill for services received at the Franklin, WI location using our HSA account card over the phone. She requested a receipt to be sent by mail with confirmation number [redacted]. After multiple inquiries on October 3 and October 9, [redacted], we were informed that the receipt had been sent, but we have yet to receive it. Despite assurances, the prolonged delay in receiving the receipt is frustrating. We are considering exploring other healthcare providers if this matter is not promptly resolved. Our principle account number is [redacted], held by James Bacik, with Carol Bacik being the patient.

I have been under the care of Tiffany K. at Ascension in Wausau, WI for two years. However, recently, there have been delays in getting my prescriptions filled. Last month, I went without medication for nine days. Today, on 12/18/21, I am facing the same issue again as my prescription has not been filled. I have contacted Ascension about this four days in advance, but the pharmacy has not received the refill. Tiffany K. is unavailable today as it is Monday, and I really need assistance with this matter.
