I submitted the necessary form, but was informed that it was unreadable. After resubmitting it without receiving a response, the issuer placed it on hold. I completed a new form with a notary three days ago and sent it in. I have been waiting for over 30 days. This delay is now preventing me from starting a new job, and I am in urgent need of it. I couldn't work yesterday as a result and risk losing the job if this continues. Can we arrange for this to be sent to me overnight to expedite the process?

Hello, I am Norina S. I recently placed an order for my son, Mario H. Jr.'s birth certificate but have not yet received it. I made the purchase on December 5th and was informed that it was shipped on the 7th. However, according to the UPS tracking site, the package was returned undelivered on the 9th. I have not received a refund or any updates on my order, which is worrying. I am concerned that I may not receive the birth certificate at all. I would appreciate it if VitalChek could arrange for a reshipment as soon as possible as it is urgently needed. Thank you.

I have been attempting to track my order for several days with no success. Tracking using my Order Number and Pin has been unsuccessful, and I have not been able to reach anyone using the phone numbers provided. I urgently need my Birth Certificate to renew my Driver's License, as it expires tomorrow on my birthday. We were born in Puerto Rico, but we currently reside in Pennsylvania. I opted for FedEx shipping to expedite the process, but as of March 9th, I am still waiting for the documents. I also included a letter from Social Security regarding benefits from my husband and my military DD Form 4. My birth certificate reflects Lucinda B. Winters, but I go by Lucy. Since the age of 14, I have been known as Lucy due to preferences and official documents. I have used Lucy B. Winters, now Dowalo after marriage, with the IRS and State of Idaho. Despite faxing my documents again, I received conflicting emails about needing more information. The reference number for follow-up is [redacted]. My order number is [redacted]67/PIN674592. My birth name is Lucinda, but I have always gone by Lucy with matching signatures and consistent documentation. I urgently need my birth certificate for a legal matter following my husband's passing. I received an email today about Order Number: [redacted]07 / PIN [redacted]. The Notarized Authorization form I faxed on March 1, [redacted], seems to be missing, incomplete, incorrect, or illegible. They mentioned the need for my signature and date in the presence of a Notary Public. To proceed, I must ensure that the Authorization form is complete and notarized with specific visible details. The email provided a link to access the updated Authorization form on the VitalChek Order Status Page. Although the VitalChek processing fee will still be charged, I need to address this issue promptly. Since January [redacted], I have been trying to update my grandfather's death certificate information through "VitalChek." The process began in December [redacted]. After making the payment, I was asked to submit an updated document. Due to the pandemic, finding a notary took some time. Once the form was notarized, every email I received from VitalChek stated that my "file/account" could not be found and requested that I call. Unfortunately, I cannot spend hours on the phone. I am looking for assistance in locating my file, reopening the account, and receiving a message with a functional number for me to scan the necessary information. I made a purchase through Vital Records for a Marriage Certificate. My order was submitted on February 16, [redacted]. After following up on February 23rd with Hennepin County Records regarding the receipt of my faxed authorization form, I was informed of a processing delay of about two weeks. On March 15, I received an email confirming that my order had been processed and provided with a tracking number. However, on March 26th, I received another email stating that my order was put on hold. Despite sending another notarized copy of the Authorization form to the fax number provided, when checking the website, it requests additional information without specifying what is needed. I paid $36.00 for expedited processing, yet no resolution. Attempts to contact Hennepin County Vital Records and the given phone number were unsuccessful. Seeking clarifications on the delay after waiting for almost two months. Had to cancel a passport appointment scheduled three weeks ago due to this issue. Eagerly waiting for updates.

I have an expired ID, and the Registry of Motor Vehicles requires a birth certificate as a secondary form to issue me a new one. I have my social security card, W2, and a letter confirming my status as a registered voter. These are the only forms of identification I possess. I recently ordered my birth certificate from VitalChek and it was supposedly delivered by UPS. However, UPS claims it was delivered last Friday, which is not accurate. Despite being home all day and having security guards at the entrance, I never received the package. The UPS tracking number is 1Z0380RE[redacted], and the order number is [redacted]57. I was informed by a UPS Customer Service Representative that it was delivered to the wrong address and that I would receive it by the end of the business day, but that never happened. I'm frustrated and concerned about the whereabouts of my birth certificate.

I have been attempting to contact someone regarding my order placed on 1/15/21 for a certified copy of my birth certificate. Upon receipt, I noticed that my middle name was incorrect. I experienced a frustrating situation where my detailed explanation disappeared after 20 minutes of writing. I applied for my New York birth certificate in January. However, every time I check my order status online, it shows an error message directing me to call Customer Service at 1-[redacted]. Good morning, I am seeking assistance with a pressing matter. As a Social Worker, I assisted a resident in ordering his birth certificate, opting for expedited delivery for disability filing on May 4th. However, despite the expected arrival on the 19th, I was notified via email of an unexplained delay. After days of trying to ascertain its status, I now received another email indicating the cancellation, again without clarification. My attempts to contact Vitals Check have been fruitless, with no response to my emails and extensive hold times on calls. Can someone help me understand this situation? The lack of information is especially troublesome as the resident urgently requires this document for disability filing. The cancellation email did not offer any reasons for the action. I lost my mother on December 22, [redacted]. After inquiring with the mortician in February about the death certificate, I was directed to contact Vital Records in Virginia. Being in Maryland, I found VitalChek online to order three Death Certificates with UPS Air on 03/08/[redacted]. As of 04/30/[redacted], there has been no communication from VitalChek. This delay is hindering my ability to deal with insurance policies and update beneficiaries on legal matters. It's been particularly challenging for me as I also lost my husband of 50 years in September [redacted]. Despite receiving an email notice about shipment delays on 03/16/[redacted], I am still waiting.
