I am requesting a full refund of $**.** due the I was sent the incorrect information on my birth certificate. I informed the custom service representative at the time I placed the order that I have a decree for name change do you need that. I was told no. I asked again as she began to take my information do I need to send you the decree of my name change. She again said no. I was not informed at any point that they will send me what is stated in the system. I stated please do not send me another birth certificate with the incorrect information. At no time during the conversation was I informed that since I have a decree name change that I should call the Virginia Health Department to place the order. I am requesting a full refund. *For the last two days I have been on hold for * *** to speak with someone. The only way I was able to get through was to state in your automatic service was to select the corresponding number to order a certificate. Once connected I spoke with Stephanie who kept telling me that it is my responsibility to know the procedure. I ask to speak with a supervisor. She placed me on hold and came back stating that I would have to contact Virginia Health Department. I stated that they stated to me that I needed to contact Vital check to request the refund due to they are the one who took the order and that they, Vital check should have informed me that I was to contact the appropriate facility to place the order. Please help. Thank you
