After my interview at Scott Sales Company on 34 Rutledge St, Nashville, TN [redacted], I felt it was essential to share a concern anonymously. During the process, I had to provide my social security number and date of birth on the application paperwork. I found this unnecessary and slightly offensive as this level of personal data should not be required during the initial interview stage. Requiring this information upfront, especially birthdates, raises concerns about potential age discrimination in their hiring process. I believe it would be more appropriate for this information to be collected once a candidate is selected for the position. I have an active unemployment insurance claim with the Tennessee unemployment office. My back pay of $9,[redacted] was rejected by the bank when they tried to deposit it on June 19, [redacted]. After being notified via email that the back pay would be placed on a debit card, I received the card but have not seen any deposits or activity on it. Even though I provided my correct personal banking information for direct deposit, I have not received the funds. Despite leaving multiple messages and submitting requests, I have not received any response. I have been unsuccessful in reaching a person over the phone. I continue to receive my current weekly benefits in my personal checking account, indicating that the correct information is on file. I hope the Tennessee Department of Labor and Workforce Development will address my concerns and allow me to speak with someone about resolving this issue.

I have contacted various times through calls, emails, and even a form submission regarding my unemployment extension issues for the months of May, June, and July. After speaking with two different individuals who assured me about back payment, I have still not received the benefits for 11 to 12 weeks. Eventually, I filed a new claim and started receiving payments for the past three weeks. However, the 12 weeks that were missed are still pending. Another representative mentioned a change in the process, stating I needed to file a new claim. I complied, but the payments for the previous months are still outstanding. When can I expect to receive those payments?

I filed on June 05, [redacted], after my last day of work on March 08, [redacted]. Unfortunately, I was incarcerated from March 15, [redacted], to April 17, [redacted]. Later, when approved for DUA, I had to certify for retroactive weeks. However, the process was challenging due to lack of live help and the changing format for self-employment applications. Despite my efforts, I mistakenly certified for weeks I was incarcerated, resulting in an overpayment notice months later. I received several notices in one day, including potential overpayment, overpaid account establishment, and determinations of fraud. Despite my attempts to contact them via phone, email, and live chat, I have not been able to resolve the issue, and they have stopped my payments. I did not intend to defraud the program and believe some blame lies with the lack of communication and complexity during the application process.

I exhausted my regular unemployment benefits in March. Despite filing my weekly claims consistently, I didn't receive any payments for two weeks. After contacting them, they mentioned delays due to high unemployment rates and assured I would receive payments within 6 to 8 weeks. However, no payments arrived for May, June, and July. Many attempts to contact them went unanswered, so I filed a new claim and finally started receiving benefits again. They informed me that a new claim is necessary after regular benefits are depleted and mentioned pending payments for the missed months. Despite this assurance, I haven't received the overdue payments yet. I'm struggling to make ends meet and urgently need the money to catch up on bills. I received a back pay of $11,[redacted] on my Insight card, similar to a NetSpend card. Unfortunately, the card rejected the payment because my full name, Miriam Dawn Daugherty, was not properly displayed, only as Dawn Daugherty. I was receiving payments for the past 4 months when they suddenly stopped because my ex-employer filed an appeal. Now, I am being told I have a lot of overpayments that I don't owe, and I am not receiving any benefits. I filed an appeal, but unfortunately, I had a stroke and needed emergency open-heart surgery. Despite this, I am definitely one of the people affected by COVID-19, and I am puzzled as to why they would remove me from receiving benefits when I am following all the correct procedures.

I require assistance in reopening my previous claim. Last Sunday, I noticed that my old claim was reinstated on my Jobs4TN.gov app, most likely by the Tennessee Department of Labor and Workforce Development. It seemed ready for updates. However, the following Monday, I expected my extension and funds to be added, but I was faced with a message on the claim stating that my certification filing window had expired with no further options provided. I was only given the choice to click a link, which unexpectedly removed my entire claim. Subsequently, I was instructed to file a new claim, which I did. Unfortunately, I have not received any payments yet, and I have not received any communication regarding the next steps.

Last week, I couldn't complete three work searches due to a hospital visit. Now, my benefits have stopped, and it's been 16 weeks since I filed. I submitted an appeal, providing proof of my hospitalization. The determination confirmed it as valid, and I received an email with an authentication code to submit my weekly work search information. I recently received five invoices in the mail claiming I owe $[redacted] in federal income taxes due to receiving $6,[redacted] in employment income. My husband suffered a stroke and heart attack right before Christmas, and my 33-year-old mentally handicapped brother passed away on January 9th due to Covid-19. We are facing extreme hardships, barely able to feed our children. I am unable to reopen my business as people are afraid of getting Covid-19. On October 6, [redacted], my issue started with not being able to file weekly certification due to a system error.
