Username, Password, Email address on the account, Address on the account, Name on the account, and Phone number on the account
The issue in GetHuman-rowkat's own words
Upon switching from DSL (plan ****) to Home Fiber (plan****) on my first billing appear a telpad service fee worth *,***.** coverage is Nov **** - to present. Please remove the unnecessary charges from my billing. **Account Name : Rowland Morales*Account No. ***********Tel no. ***-*********Hope for your fast response unlike the *** hotline tried * times and waited for more than **minutes but to no avail.

GetHuman-rowkat did not yet indicate what PLDT should do to make this right.
