Hello my name is Willy. When I originally purchased a JUUL I did the survey and received my gift card no issues. Then I received my monthly survey for * more months. And hadn't received a survey for November or December. I have bought over a dozen new device kits and spend at least $*** a month on products of JUUL so the $** gift card is a great incentive to stay with JUUL. I just went and started a new survey at www.juul.com*survey and put in a new Access Code I saved from a new device I bought (**E*-DACD) and started the survey and the page then closed. I couldn't reopen it so I tried to re enter it but it will not work. I would appreciate a prompt response if possible. I enjoy JUUL products but would like to also receive my gift cards for being a loyal customer. I look forward to hearing back from you soon. Thanks again, * Willy Gwaltney
