My mother died in April ****, and because of the coronavirus many of the government agencies here in, Puerto Rico, where closed. My lawyer was able to request a copy of the death certificate about a month and a half ago. My agent had originally request the death claim as I am the beneficiary, however since the certificate was not available for several months the hospital certificate was submitted. The request was denied until the death certificate would be sent. I have tried for two weeks to forward a copy of the certificate by phone to my agent since the office is closed, but the document does not come out clear according to my agent. I requested if I could fax but the document could not be transmitted because the fax machine in the agent’s office, here in San Juan, PR was not working!! I have been trying unsuccessfully to submit this document. Can you forward the claim form so that I can submit everything by certified mail.
