My husband passed away on March 1, [redacted], and I was named as the beneficiary of his $50,[redacted] insurance policy. However, when I filed the claim, the insurance company informed me that I was not listed as the beneficiary. Despite providing them with the necessary paperwork, including the voluntary administrator form and a copy of the policy with me listed as the beneficiary, the check I received was made out to the estate of Roger Marvin. I set up an account under that name to cover medical and funeral expenses. Now, I've been told I need to file a Form [redacted] for taxes. I believe insurance payouts are not taxable to the beneficiary, so I'm concerned about any potential tax implications due to the check being made out to the estate. Despite verifying my address, I still did not receive the required document. The IRS suggested that having a PO Box might be the reason for the delay. They advised me to visit the nearest IRS office, which for me is either in Redding or San Francisco, both of which are hours away. Instead, I decided to wait until this year when I filed my taxes at H&R Block. However, they informed me that since the previous year's filing was received by the IRS, I would need to resolve the issue with them directly.

I am currently struggling with heart failure and falling behind on bills. I am working from home to catch up but had to resort to high-interest loans to stay afloat. By committing to paying a thousand dollars monthly, I hope to resolve this sooner. I take 12 medications daily and am hopeful for a valve fix this winter to alleviate my health issues. Despite my attempts to access funds from my retirement, I was unsuccessful. I kindly ask for some leniency during this challenging time, as losing my job or checking account would be detrimental. I have been unwell for years and rarely leave my home aside from work. I am willing to sell possessions to catch up on car and house payments if given a few months to gather the necessary funds. My name is Patricia A. Merrick and I had been in touch with the IRS a couple of weeks ago. I received one letter from them, but the crucial agreement they were supposed to send hasn't arrived. Last week, I received a notice from the post office to collect some mail, but due to my work schedule, I haven't been able to pick it up as they close before I finish work. I visited the post office today on my day off, but they had nothing for me. I recently underwent an audit and realized I mistakenly sent my documents to the wrong state. After speaking with your team on 9/17, I resubmitted the necessary documents on 9/18 via fax. I am contacting you to inquire about the status of my audit. The audit was triggered by an error in reporting my taxable IRA as verifiable income instead of a taxable IRA. A few months ago, I reached out to Get Human regarding my tax return filed through the H&R Block app on Google Play in February. Since then, I haven't heard anything from the IRS regarding my refund. When trying to check online, the website asks for the exact refund amount, which I enter but am met with an error message stating it's incorrect. H&R Block has informed me that they are not accountable as the app came from the Play Store. I am at a loss and in need of assistance as I can't seem to find anyone who can provide guidance on this matter. Any advice would be greatly appreciated.

Hello, my name is Terry Parsley. I am encountering an issue with my employer at Washington Square Mall where I work in security. The mall has recently been acquired by a new owner. Upon receiving my first payment from the new owner in the form of a company check without a check stub, I noticed the amount was incorrect. I misplaced Shirley and Jerry O'Malley's [redacted] W2 forms. I need replacements as I don't owe the IRS. Unfortunately, I lost them when my husband was in the hospital and passed away in October [redacted].
